On the way to phase 12 of the mine pit, the operator of a CAT 793 haul truck collided with the central safety
Description of
berm (tire), damaging the ladder and small parts of the truck.
the incident:

• The Operator reports that he believed that he slept and woke up with the impact of the truck on the
Immediate tire.
• Refreshment training for the operator
Action Taken: • The DSS was checked in the truck after the accident and it was working.
• The investigation is ongoing checking all information on the fatigue system
