AFFIDAVIT OF ONE AND THE SAME PERSON
I, CEFERINO BARTOLOME Z. BINAYUG, of legal age, Filipino citizen,
married, and a resident of P2 BUNTONG PALAY, SILANGAN SAN MATEO, RIZAL,
after having been sworn in accordance with law aver that;

1. I am the child of GERARDO BINAYUG and MARIA ZALUN BINAYUG;

2. I have used the name CEFERINO BARTOLOME Z. BINAYUG, in all of my

records and dealings such as government ID’s, and others of similar nature.

3. My all my government ID’s such as SENIOR CITIZEN ID states my first name as

“CEFERINO BARTOLOME” to which my identity is based. As such, I have
used the name “CEFERINO BARTOLOME” while in my Birth Certificate is
“RUFINO” instead of “CEFERINO BARTOLOME”which is the name appearing
in all my government ID’s and school records.

4. I am under the impression and belief that “CEFERINO BARTOLOME” is my

real and ONLY name;

5. It was only during the time that I need to secure requirements for my
PASSPORT APPLICATION that I discovered that a name “RUFINO” appeared
in my Birth Certificate instead of the name “CEFERINO BARTOLOME” which I
am using since childhood.

6. I am executing this affidavit to attest that “RUFINO BINAYUG” as appearing in

the copy of my Birth Certificate and “CEFERINO BARTOLOME Z. BINAYUG”
as appearing in the copy of my GOVERNMENT ID’S are ONE AND THE
SAME PERSON.

7. I am executing this affidavit to attest to the truthfulness and honesty of the

foregoing facts and for all intents and purposes it may serve.

WE, CONCHITA A. LOYOSA, of legal age, Filipino and a resident of

GLORYVILLE PHASE 2 BUNTONG PALAY, SILANGAN SAN MATEO, RIZAL, and
AMELITA ERO NUÑEZ, of legal age, Filipino citizen and a resident of PUROK II
BUNTONG PALAY, SILANGAN SAN MATEO,RIZAL, after having sworn to in
accordance with law, hereby depose and say:

1. That we know personally CEFERINO BARTOLOME Z. BINAYUG a.k.a.

RUFINO BINAYUG who was born on AUGUST 24, 1951, Filipino
Citizen, of legal age and single;

2. That we know him to be using as his name CEFERINO BARTOLOME Z.

BINAYUG in all his private and public dealings instead of RUFINO
BINAYUG his true and correct name appearing on his Birth Certificate;

3. That CEFERINO BARTOLOME Z. BINAYUG and RUFINO BINAYUG

is one and the same person;
