AFFIDAVIT OF EXPLANATION

I, JEAN, Filipino, single, of legal age, with address at Lapu-lapu

City, after being sworn in accordance with law, do hereby depose and state
that:

1. I am the mother of JOHN who was born on June 5, 2002 at Cebu

City and whose birth was registered under Local Civil Registry No.
2002;

2. In the Certificate of Live Birth of my daughter however, her gender

was inadvertently registered as MALE instead of her true and correct
gender which is FEMALE as appearing in her Certificate of Live
Birth;

3. The reason for the error in her maiden name was the inadvertence
of the clerk who typed in the details in my daughter’s Certificate of
Live Birth;

4. I am executing this affidavit to explain the error in the birth record

of JOHN as regards her gender and for whatever legal purpose this
may serve.
