AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

BEING EXEMPTED BY LAW

I, CARLITO BADION, of legal age, married, Filipino, Blk 14, Lot 74 , Kasiglahan
Village I,Roddriguez,Rizal, on oath, deposes and says that :

1. I am the father of ESTHER ANNE CABRILLAS who is a student and a scholar

in Papaya Academy, Kasiglahan Village, Rodriguez,Rizal ;

2. One of the requirements in the said school and scholarship program is the
submission of the individual income tax return ;

3. Under Section 44 (2-A) of the Tax Code of the Philippines as amended herein,
affiant is exempted and is not required to file any income tax returns as long as
he/she has no income and overseas Filipino Workers (OFW) which he/she
derived solely from wages does not exceed the Personal Exemptions , Annual
Income not exceeding to P20,000.00 ;

4. I am presently no income whatsoever;

5. I did not file my income tax returns for the year requested pursuant for the
aforementioned provisions of the laws ;

6. I would like to request for a certification attesting to any exemption for income
taxation ;

7. I am executing this affidavit to attest the truth of the foregoing facts and for
whatever legal intents and purposes this may serve .
