AFFIDAVIT OF UNDERTAKING

I, Rachel T. Alegado, Filipino, of legal age, with permanent address at Block 11, Lot 3,
Camella, Valle Cruz, Cabanatuan City, Nueva Ecija, after being sworn in accordance with
law, hereby depose and state that:

1. I am a grantee of the Scholarships for Graduate Studies - Local under the CHED K to 12
Transition Program.
2. I am/have submitted my request for Leave of Absence to the Commission in relation to
the grant as stated above.
3. I certify that the information indicated in the documents I have submitted in relation to my
request are valid, authentic, true, and correct based on my own personal knowledge and
based on documents in my possession.
4. I understand that the request shall be subject to due process and may be approved or
disapproved based on the deliberations of the Commission.
5. I understand and am willing to comply with the conditions of the decision made by the
Commission, and shall sign a contract reflecting the same.
6. I understand that the Commission may rescind any agreements and take the appropriate
actions should I be found to have violated any of the conditions of the decision.
7. I am cognizant, willing, and accepting of the commitments that shall follow from my
request, in accordance with the policies of the scholarship, including those to be issued
in the future.
8. I fully understand the consequences of not giving correct and truthful data and I am
aware that non-disclosure of material information as required will put me under the pain
of perjury by executing this document.
