JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ISMAELITA T. MOSQUEDA, Filipino, of legal age, married and a resident of Tip-Tip
Distrcit, Tagbilaran City, Bohol, Philippines, and ELENA C. PITOY, Filipino, of legal age, single and a
resident of Remolador Street, Tagbilaran City, Bohol, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say:

That we both personally know the person of the late CINDIO DANO DIGAO who was born to
spouses IGNACIO DIGAO and SOLIDAD DANO DIGAO on July 11, 1942 at Sevilla, Bohol,
Philippines;

That CINDIO DANO DIGAO died on October 4, 2016 at Poblacion, Sevilla, Bohol, Philippines;

That the following discrepancies appeared in some of his documents, to wit;

CENDIO DANO DIGAO – as per his Social Security System Salary Loan Application with
Batch/Item No. 167728;

CINDIO DANO DIGAO – as per his Birth Certificate issued by the Office of the Local Civil
Registrar of Sevilla, Bohol, Philippines;

That by virtue of these presents, we hereby attest that the names, CENDIO DANO DIGAO – as
per his Social Security System Salary Loan Application with Batch/Item No. 167728, and CINDIO
DANO DIGAO – as per his Birth Certificate issued by the Office of the Local Civil Registrar of Sevilla,
Bohol, Philippines, all refer to one and the same person, that is the person of CINDIO DANO DIGAO
who was born to Spouses IGNACIO DIGAO and SOLIDAD DANO DIGAO on July 11, 1942 at Sevilla,
Bohol, Philippines;

That we are not in a way related either by consanguinity or affinity to the said person and neither
are we interested in the outcome of this affidavit our purpose being merely to set the records straight;

That we are executing this affidavit not for purposes of fraud but to prove the real name of
CINDIO DANO DIGAO and for whatever legal purpose this may serve.
