    List of Requirements for Application of Hazardous Generator’s

    Identification

    (HWG ID No.)

    1. Transmittal Letter;
    2. Accomplished and notarized application form;
    3. Pollution Control Officer (PCO) Accreditation Certificate;
    4. Description of existing waste management plan signed by PCO;
    5. Process flow diagram of all waste streams signed by PCO;
    6. Copy of Environmental Compliance Certificate (ECC) or Certificate of
    Non-Coverage (CNC) for the project;
    7. Copy of valid “Permit to Operate” for air pollution source installation,
    if applicable; and
    8. Copy of Valid “Discharge Permit” for wastewater, if applicable.

    Note:

    Registration Fee = P600.00

    Amended of Registration = P600.00

    Documentary Stamp Tax P15.00
