A. PERSONAL/PROFESSIONAL
 Conducted an outreach program and storytelling project of BU ASLIS last September 22,
2012 in Bethlehem House of Bread, Poblacion, Baliwag.
 One of the committee of National Book Week Celebration last November 26, 2012.
 Assisted Ms.Lynderlitte M. Maglaque in preparing documents for accreditation.
 One of the judge in Essay Writing Contest during National Book Week Celebration last
November 26, 2012.
 Assisted and supervised the On-the-Job trainees in cataloging and classifying library
materials.
 Supervised and evaluated the performance of the student assistants that was assigned
in Technical Section.
 Facilitated the annual team building of BLIS students last December 18, 2012.

B. PHYSICAL FACILITIES
 Coordinates with the IT technicians regarding the maintenance of the computers.
 Coordinates with electrician for the maintenance of lighting, broken wires, etc.
 Evaluate janitors in-charge in the technical section.

 Cataloged, classified and encoded the purchased and donated books, theses and
electronic resources.
 Prepared receiving copy of the encoded library materials.
 Distribute the encoded library materials to its respective sections.

 Appointed as Assistant Chief Librarian May 2013.
 Served as coach of the 2 representatives for the 22 nd LIS Wizard Competition held at UP
Diliman last November 29, 2013.
 Part-time instructor of the BLIS Department under the College of Education.
 Finished a research study with Mr. Elvin R. Ventucillo entitled “The Baliuag University
Main Library Services Users’ Satisfaction Survey: the Students’ Perspective” for SY 2010-
2011.
 Member of Regional Quality Assessment Team (RQAT) for the BLIS Program via CHED
RO-III.
 Appointed as Board of Directors of PATLS for the year 2014-2015.
 Renewed membership on PLAI, CLLA and PATLS valid until 2015.
 Facilitated the bibliography and document generation of the Library for the MBA Level III
Reaccreditation, BS Psychology Preliminary Visit held last March 2014.
 Supervised the ASLIS National Book Week Celebration Activities (Essay Writing, Poster
Making, Photo Exhibit, INFO Hunt, and Shelf-reading contest) held last November 25-29,
2013.

B. PHYSICAL FACILITIES
 Coordinates with the IT technicians the maintenance and hardware operability of the
Library System and OPAC terminals of all libraries and sections.
 Coordinates the access implementation of the Wi-Fi services of the Main Library and
satellites.
 Coordinates with the EISI the maintenance and hardware operability of the Library

 Responsible for creating back-up files of the Library System.
 Revised the floor plan of the library building.
 Coordinates with research center and outside designer for the renovation of the library
building.

C. PUBLIC SERVICES
 Supervised the over-all day to day operation of Information Commons.
 Oversaw and evaluated the performance of student assistants in the performance of
their duties.
 Assisted faculty and students in using the OPAC, E-Journals and other computers
applications.
 Provide List of Newly Acquired Materials for the requesting faculty members used for
syllabus making.
 Provide and update the List of E-Journal Titles Subscription by Colleges.
 Coordinates with the different deans of colleges for the selection of e-journal titles.
 Finished the Guide to Movies vol. 1
 Edited the Guide to Movies vol. 2
 Revised the Library Policy Manual, Library Handbook, and Policy Manual of Information
Commons.
 Design the website workflow and website contents.
 Submitted three (3) articles for Horizon and four (4) articles for Horizon April 2014 issue.
 Proposed a layout for the official library website.
 Create a movie wall displays as a decoration for the Multimedia Kiosk.
 Responsible for updating the content of the INFO Bulletin Board at the Information
Commons.
 Designed a brochure for the Baliwag Studies Center, University Archives and Guide on
how to use E-Journals.
 Designed the Request Form for the Official Use of Library Facilities.
 Orient the newly hired Student Assistants for SY 2013-2014.
 Summarized daily statistics on users of Information Commons for the SY 2013-2014.

D. PROBLEMS
The library is in the ground floor and at the 2 nd floor, the students’ classroom, the ceiling that
connects the 1st and 2nd floor was made of wood, the problem is you can hear the stomping of
students’ feet and it distracts the library staffs and researchers because of the noises it created.
It is also dangerous because there is a possibility that the ceiling might fall down also the lights
and ceiling fan
Wooden shelves are some termite infested that it will be dangerous to the collections. It is
necessary to replace them with steel shelves, if not possible repair and repaint those wooden
shelves to preserve library materials.
Now, the new library location is small, inadequate of space for new collections and the users.
Inadequate of shelves for newly acquired books.
E. RECOMMENDATIONS
To attend more profession related seminars 
More seminars to attend to keep track of the latest trends and issues regarding cataloging and
classification.
Change wooden shelves into steel shelves, if possible. 
The library need more space for the future collections and users.
Add more steel shelves.
