Dear Sir/Ma’am:

This is with regard to attendance on the Unified Licensing Seminar (ULS) offered by the FDA
for 2019 as required in the FDA License to Operate (LTO) application. I tried to register based
on the given process and did submission a couple of times however registration was not
successful due to seminar is already full and no slots are available.

With this, I am aware that once there will be an available licensing seminar will be offered, I shall
attend the said seminar and comply whatever requirements as required in the FDA License to
Operate (LTO) application.
