UPRHS ENTRANCE EXAMINATION 2019

WHAT IS THE UPRHS ENTRANCE EXAMINATION?

The UP Rural High School (UPRHS) Entrance Examination is a 3-hour and 20-minute comprehensive examination on the following
areas: Language Proficiency, Reading Comprehension, Mathematics and Science.

For admission in the school year 2019-2020, the Entrance Examination may be taken only on December 1, 2018 (Saturday), in two
sessions: morning (8:00 – 12:00), and afternoon (1:00 – 5:00). The venue of the test is at the CAS Annex 2, UP Los Baños, College,
Laguna. NO OTHER DATE IS SCHEDULED FOR THE EXAMINATION.

WHO ARE QUALIFIED TO TAKE THE UPRHS ENTRANCE EXAMINATION?

Qualified to take the UPRHS Entrance Examination are applicants who have:
1. completed grade 6, or are candidates for elementary school graduation as of the end of the
school year 2018-2019;
2. not previously taken the UPRHS Entrance Examination;
3. not attended high school or Grade 7;
4. submitted ALL documents required for processing of the application; and
5. obtained the Test Permit for the UPRHS Entrance Examination.

WHERE CAN APPLICATION FORMS BE OBTAINED?

Application forms for the UPRHS Entrance Examination are available at the UPRHS Office of Admissions and Registration, Paciano
Rizal, Bay, Laguna, and the UPLB Registrar’s Office, starting October 5, 2018.

WHEN AND WHERE IS APPLICATION FORM FILED?

Application forms may be filed from November 5 to 29, 2018, from Mondays to Fridays only, at 8:00 to 11:00 in the morning and
from 1:00 to 4:00 in the afternoon at the Office of Admissions and Registration, UPRHS, Paciano Rizal, Bay, Laguna. ONLY
APPLICATION FORMS WITH COMPLETE SET OF REQUIREMENTS WILL BE ACCEPTED FOR PROCESSING.

NOTE: ALL DOCUMENTS FILED IN SUPPORT OF THE APPLICATION BECOME THE PROPERTY OF
UP RURAL HIGH SCHOOL, AND WILL NOT BE RETURNED.

WHO WILL QUALIFY FOR ADMISSION TO UPRHS?

Examinees who rank among the top 125 in the UP Rural High School Entrance Examination are the Qualifiers for admission. In the
event that some of them choose not to enroll in UPRHS, the slots that they will vacate will be given to those in the Waiting List in the
order of their ranking until all 125 slots are filled up.

WHAT SHOULD THE APPLICANT DO IN CASE OF LOST TEST PERMIT?

Before the Examination date, the applicant must request from the UPRHS Office of Admissions and Registration for a replacement of
the lost Test Permit. For this request, he/she must submit one 2” x 2” picture identical to one attached on the submitted
application form.

REMINDERS TO APPLICANTS:

1. Every applicant must be in his/her respective examination room 30 minutes before the time specified on the Test Permit.
2. Applicants are also advised to bring their snacks with them as they enter the examination room. Once the
examination has started, Examinees will NOT be allowed to go out except for personal necessities. Likewise, NO snack pack
will be received for them while tests are going on.

NOTES TO PARENTS/GUARDIANS:

1. Applicants with contagious diseases (such as chicken pox, measles, mumps, etc.) will not be allowed to take the
examination.
2. Parents/guardians will not be allowed to stay in the CAS Annex 2 building premises while examination is going on. They
are requested to leave the area 30 minutes before the start of the examination proper.

Application Procedure

1 STEP 1: Complete the Application Forms


Get, complete, and submit the following application forms and requirements.
• UPRHS Entrance Examination Application Form (UPRHS AF1)
• Certified true copy of Permanent School Record (Form 137) w/ Grade V
& Grade VI grades (at least for the first two quarters)/ Form 138
• Two (2) recent identical 2”x2” pictures
• Completed form of Confirmation of Enrolment and Report of
Grades (UPRHS AF4)

Note: Any Filipino applicant whose family has a combined annual gross
income of P80, 000 or less is exempted from paying the application fee.
He/ She must submit a Certificate of Indigency.

STEP 2: Submit Application Forms
• Visit the UPRHS Office of Admissions and Registration to submit your
requirements. A representative may do this step for you.
• Prepare a non-refundable application fee of P350 for Filipinos or P700
for foreign nationals/resident aliens.
• Get your UPRHS Entrance Examination test permit

STEP 3: Take the UPRHS Entrance Examination
• Bring the following:
✓ UPRHS Test Permit (No test permit, no exam);
✓ Two (2) pencils; and
✓ Light snacks.
• Be at your respective examination room 30 minutes before exam time
