REQUIREMENTS FOR REGULAR MEMBERSHIP:

1. A duly licensed physician to practice medicine in the Philippines

2. A Member in good standing of the Philippine Medical Association
3. Physician must have completed satisfactorily a post-graduate course in Occupational Health
given by the college, by a medical school or institution duly recognized by the government
or by the College of Public Health of the University of the Philippines
4. Must have been in active practice of Occupational Medicine as defined by the College
whether in private or government service, on full time or part time basis, for at least two (2)
years

REQUIRED DOCUMENTS:
1. Photo copy of Certificate of Post-Graduate Course in Occupational Health and Safety/Basic
Course in Occupational Medicine
2. Resume with colored 2” x 2” Photo.
3. Certificate of Good Standing from PMA
4. PMA and PRC ID Card (Photo copy)
5. Payment of P1,750.00 (Entrance Fee = P1,000.00; Annual Dues = P500.00;
Mutual Aid; P200.00; ICOH Fee = P50.00 (Local Dues not yet included)
