JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, JAIME I. LABABIT and ADELA A. DIEGO, of legal ages,

Filipino citizens, and residents of Barangay Carael, Camiling, Tarlac,
Philippines, under oath, do hereby depose and state THAT:

1. We both personally know RICARTE BERSIDA DELOS SANTOS

(Ricarte, for brevity), who was born on November 7, 1932 to Spouses
Esmeragdo Delos Santos and Celestelita in Camiling, Tarlac, Philippines;

2. On January 10, 1953, Ricarte got married to Eufemia Agustin at the

Justice of Peace, Camiling, Tarlac. However, on February 18, 1964,
Eufemia passed away, thus, Ricarte became a widower;

3. In 1968, Ricarte met Teodora Mandia (Teodora) and they lived together as
husband and wife without the benefit of marriage until they decided to
marry each other on June 2, 1994 before the Office of the Municipal
Mayor in Camiling, Tarlac;

4. Ricarte is a member of the Social Security System (SSS) until his death on
March 15, 2016 in Camiling, Tarlac. When Teodora, (Ricarte’s wife)
applied for the Death Claim Benefit of her late husband from the SSS, she
was required to submit copies of Ricarte’s Birth, Marriage, and Death
Certificates, Advisory on Marriages from the PSA, and the Certificate of
Death of Eufemia;

5. Teodora was able to secure all the above-required documents, and upon
verification of their contents, it was discovered that in the Certificate of
Death of Eufemia (Ricarte’s first wife), her first name recorded thereon is
EUFEMINA;

6. Moreover, in the marriage contract of Ricarte and Eufemia, the name of

Ricarte’s mother appears as CELESTINA BERSEDA, while in the
Advisory on Marriages issued by the Philippine Statistics Authority it is
CELESTILITA BERSIDA, when in fact, her true and correct name is
CELESTELITA BERSIDA;
7. On the other hand, the first name of Ricarte’s father appearing in his
certificate of birth is ESMARAGDO instead of ESMERAGDO;

8. Furthermore, in the certificate of death of Ricarte, his middle name was

recorded as BERCIDA instead of his true and correct middle name
BERSIDA. Hence, the discrepancies;

9. We are not related to Ricarte or to his family by affinity or consanguinity

except that of being his neighbors and family friends;

10. We are executing this Affidavit to attest to the fact that the names
EUFEMIA AGUSTIN, appearing as the first wife of Ricarte Delos
Santos in the above-mentioned marriage contract and Advisory on
Marriages, and EUFEMINA M. AGUSTIN, appearing in her certificate
of death, and EUFEMIA AGUSTIN DELOS SANTOS, refer to one and
the same person and that is the first wife of our neighbor and friend
Ricarte;

11. Also, we are executing this affidavit to attest to the fact that the names
CELESTELITA BERSIDA, appearing as the mother of Ricarte Delos
Santos in his certificate of birth, CELESTINA BERSEDA appearing as
the mother of Ricarte Delos Santos in his marriage contract with Eufemia
Agustin and CELESTILITA BERSIDA, appearing as the mother of
Ricarte Delos Santos in the Advisory on Marriages issued by the PSA,
refer to one and the same person and that is the mother of our neighbor
and friend Ricarte;

12. In addition, we are executing this affidavit to attest to the fact that the
names ESMARAGDO DELOS SANTOS, appearing as the father of
Ricarte Delos Santos in his certificate of birth and ESMERAGDO
DELOS SANTOS, appearing as the father of Ricarte Delos Santos in his
marriage and death certificates, refer to one and the same person and that
is the father of our neighbor and friend Ricarte;

13. Furthermore, we are executing this affidavit to attest to the fact that the
names RICARTE BERSIDA DELOS SANTOS as recorded in his
certificate of birth and RICARTE BERCIDA DELOS SANTOS as
appearing in his certificate of death, refer to one and the same person and
that is our neighbor and friend;
14. Finally, we are executing this affidavit to attest to the truth of the
foregoing statements and for whatever legal purposes and intents it may
serve.
