AFFIDAVIT

I, JANICE JESULGA JANGAD, Filipino, of legal age, married and a resident of Door 2
Blue Apartment, Sitio Atis, Banilad, Cebu City, after being duly sworn to in accordance with
law, hereby depose and state THAT:

1. Shun Richard Thompson, an Australian citizen, a resident of Brisbane,

Australia, and married to my sister, Jackielou Thompson, invited me to visit
their homeplace;

2. My sister, Jackielou Thompson, has facilitated my tourist visa application for

Australia;

3. I have no cash deposits to any banks in the Philippines. Hence, I cannot

submit the required bank statements;

4. Other than my compensation, I rely on my husband, Francois Vir Jangad, who

is now in Thailand and my sister, Jackielou Thompson, for the chief support
of my five (5) children;

5. Should my tourist visa application be granted by the Australian Embassy in

the Philippines, I authorise, assign and entrust the care and custody of my
five (5) children to my mother, Marilou Jesulga, and to my aunt, Vicenta
Gerona; and

6. I am executing this Affidavit to attest to the truth of the foregoing facts, and
to fulfil my requirements in my application for tourist visa before the
Australian Embassy in the Philippines.

IN WITNESS WHEREOF, I have hereunto set my hands this 8th day of June 2018 at
