26. The temperature being high in the area

INSPECTION REPORT OF ADNOC OFFSHORE UZ750 PROJECT (NORTH ISLAND)

INTRODUCTION

ADNOC Offshore is the leading Exploration, Oil and Gas production and Supply company is the state-
owned Oil company of UAE. It is the world's 12th largest oil company by production, standing at 3.1
million barrels per day. It is the UAE's biggest company. North Island is one of the 4 Artificial Islands
(North Island/ South Island/ Central Island and West Island) in UZ Field located near existing central
complex and satellites and it is 101 Km away from the mainland. The four islands are oval in shape
and measure 2,000ft to 2,600ft in diameter. The Surface Facilities consists of Process (Production,
Separation, Compression, Water Treatment, Water Injection and Power Generation) and Non-
Process Facilities (Accommodation, Warehousing, Offices, Workshops, Helipads and General
Services). Layout of the North Island includes 184 slots of well (oil production as well as water
injection wells). It also has 238 km Pipelines and 127 km Subsea cables connecting all the 4 Islands
and existing facilities with Brownfield Projects at UZ-Central Complex and Satellite Platforms.

On 6th November 2017, I conducted the work place inspection in North Island. The main purpose
was to carry out a safety inspection and identify the hazards and evaluate the existing control
measures and to suggest any additional control measures if needed. During the inspection I found
various unsafe conditions and few numbers of good practices also. The site has a man power of 1000
personnel’s which includes workers from various contracting companies as well as the client who
belongs from different parts of the world. The company has a 24hrs work pattern and the employees
are working in three shifts. In case of extended hours work by the employee, employer is providing
over time. Employees belonging to various trade category like construction workers, managers,
supervisor welders, crane operators, plumbers, painters, electricians, engineers, supervisors, NDT
Technicians, helpers, drivers and other supporting staffs are working in North Island. The main
equipment’s that I observed in the site are the Crawler crane, Mobile crane, Tipper, Ambulance
vehicle, Transformers, Christmas Tree, Separators, Excavators, Welding machines etc. The company
has a good safety. The management and staff have a positive attitude and they were following and
implementing the site safety rules., painters, plumbers, safety specialists, engineers, managers,
supervisors, loading and unloading employees, housekeeping staffs and other contractor workers.
The work that is being carrying out in the company includes activities on live electrical systems, work
at height, Driving, manual handling, working with power tools and machinery maintenance works,
drilling, grinding, construction of new buildings, maintenance work of old buildings. Common
equipment’s and vehicles which are being used in the company includes JCB, MEWPs, Water
Tankers, Sewage Trucks, Dumper Trucks, Drilling machine Grinding machine, Tools for electrical
work, Fork lift truck, cement mixer, Tipper lorry, passenger vehicles for the staffs and management
etc.
EXECUTIVE SUMMARY

On conducting the health and safety inspection in this company, it was found that the overall health
and safety standard of this company is good. The organisation is providing adequate information
regarding the hazards present in the workplace and are also providing a basic control measures for
the visitors and welfare facilities are appreciable. At the same time, I could notice lots of negative
practice related to work and safety and environmental issues. Throughout the inspection all team
members actively participated in the entire inspection process. Positive approach was noticed
towards Health and Safety by all the participants while briefing the recommendations to the
participants. There is proper provision of Clinic with bed, fan, stretcher, first aid box in the North
Island. An arrangement of two ambulance for site is provided, which will help at emergencies to
transport the injured to clinic at earliest and there is also provision for Medivac to fly emergency
cases to the Onshore Multi Speciality Hospitals. The company has a good and robust heat stress
management program. The company strictly follows “No Eating in Work place policy” to prevent
biological hazards from occurring. The company provides induction program in 3 different languages
to ensure that the work force understand it properly. Tool box talk has been made a compulsory pre-
cursor prior to starting any work.
This report focuses on major principal areas from the workplace that require immediate
attention. I would like to suggest the management to adopt the control measures as soon as possible
by doing a proper assessment on issues. These major areas are listed below in order of priority and
importance. More detailed information is provided in the main body of this report, which includes
timescales, approximate costs, Laws and standards related to the issue etc.
The major findings from the workplace that require immediate attention includes:

Fire and Rescue: This is an area which needs considerable attention during the inspection I saw that
workers were carrying out hot works close to oil well without doing gas tests and carrying out
welding activity in hazardous area with minimum or no personal gas monitors with them, there was
also an improperly maintained and obstructed fire exit route in the Substation of the Company.
These activities cause fire in the workplace and will affect to both people, machines/equipment’s
and structures in the company. This will lead to a huge amount of cost to the company which will be
burdened with direct and indirect cost involved as an after effect of the accident. By providing
enough quantity of PPE’s, Personal monitors and Portable gas detectors and by maintaining the fire
exit routes properly and providing enough firefighting equipment’s are the control measures for the
fire.

Electricity: Working on live electrical systems in the company is at a dangerous situation and it was
found that there were some damaged cables with improper protection and the workers were
carrying out electrical work without using proper PPE thereby increasing the risk of electrical shock.
The workers may get exposed to electricity and may get electric shock or electric burns. By replacing
the damaged cables, provision of adequate PPE and by controlling the works underneath live
overhead power lines and providing adequate training are the remedial actions identified.

Confined Space: This is another important area which needs proper attention. I found a group of
workers working inside a confined space carrying out welding activity on carbon steel pipeline. They
were carrying out the activity without conducting gas tests, lack of personal monitor was observed.
This pose a serious danger as there is no means to find out if there is any presence of hazardous gas
mixture inside the confined space which can cause fire or explosion leading to serious burn injury or
even fatality. There was no rescue equipment available at the location and neither was the permit
accompanied with a rescue plan. This pose a serious risk to life as valuable time will be lost in trying to
get the rescue equipment’s on site and this could mean the difference between the patient surviving
or dying. This will negatively affect the health and safety culture of the organization.

Lifting: Another major issue that I could found was unsafe lifting activity and improper lifting
procedure and safety norms being followed improper lifting procedure can create imbalance in the
centre of gravity of the load and the load will become uncontrollable which might swing and hit other
structure, the lifting sling might snap, and the load can fall on the crane or people working nearby. I
observed that in the drilling workshop workers were lifting heavy drilling pipes manually and placing it
on the low bed truck even though HIAB Truck was available as there was shortage of trained operators
who could operate the lifting equipment in the process the workers were subjecting themselves to the
risk of getting Musculoskeletal disorders, WRULD, Hernia etc.

Biological: This is another important area where the company needs to give more importance. I
observed that in the senior mess hall foul smell was coming from underneath the mess hall due to
stagnant water accumulating as the drain pipe was leaking contaminated water on to the ground
and excess water had accumulated underground. It may lead to spread of Legionella and Leptospira
bacteria it can cause health effect of legionaries and leptospirosis diseases. It also serves as a good
breeding area for the mosquitoes and thereby increasing the chances for health problems like
chicken guinea.

As per the article 10 of ILO R164 and C155 , Abu Dhabi Federal Law N.O: 8 of 1980 it is the duty of
the employer to ensure that the workplace, work activities, work equipment’s and substances are so
far as is reasonably practicable and without risk to health of its employees. It is also their
responsibility to provide of training and necessary supervision. The amount the company will be
spending for the implementation on these control measures will help them in the future by reducing
the accident and incident rates. Good health and safety practise often relies on the perfect
behaviour of the individual and this depends upon the safety culture that is prevailing in the
organisation. The management plays an important part and they need to show visible commitment
and leadership in the management of health and safety related issues which will help improve the
overall moral of the employees. Safety will become an important aspect and priority in their work as
well as in their everyday life activities. This will help in creating a responsible human being. I hereby
suggest the management to adopt the control measures as soon as possible by conducting a proper
assessment on the issues. These major areas of concern are listed below in the order of importance
and priority. More detailed coverage has been provided in the main body of this report, which
includes timescales and approximate costs. Laws and standards related to the issue etc.
MAIN FINDINGS OF THE INSPECTION

1. FIRE AND RESCUE (Observation No. 4,14, 17)

Fire hazard is one of the major concern and is a serious issue in the company. During the
inspection it was noted that hot works such as welding is being carried out near live oil well without
proper gas test and a valid permit, any spark from the welding process which may get in contact with
the oil will causes fire and it may be spread by direct burning, conduction or convection or radiation.
The fire exit routes of the Substation were not maintained properly, and the firefighting equipment’s
were not displayed. This will cause enormous damage to the building and equipment’s. People were
found to be smoking in the open areas this is not only harmful to the person who is smoking but also
to the people who inhale the exhaled smoke (passive smoking) and this may cause respiratory disease
and cancer if smoking is continued on a long-term basis. This also pose fire hazard if unextinguished
cigarette bud falls on dry grass, paper or oily substances. Items that are not directly destroyed by the
fire will often be severely affected by smoke damage. It is observed that in most of the work place
fire incidents people are not killed by the fire directly instead they die due to suffocation by inhaling
smoke. People will also suffer from serious burn injury. The company may face various direct and
indirect cost such as hospital expenses, fines in criminal courts, compensation, enforcement action,
loss of good etc which will arise due to the accident.
As per the article 10 of ILO R164, UAE Federal Law N.O:8 of 1984(UAE Offshore law), it is the
responsibility of the employer to provide safe place of work, safe plant and equipment. The
observed risks are in violation to the laws of the international labour organisation. UAE Ministerial
Resolution N.O 32 of 1982 mentions that the employer must protect employees from occupational
hazard and illness.
To avoid fire, I recommend improve housekeeping, supervision, Training. Provide designated
smoking area. Provide signage’s such as no smoking area and posters to highlight the ill effects of
smoking. Provide proper permit to work training and appoint a competent permit coordinator. Ensure
there are enough quantity of Fire extinguishers available. The estimated cost for the implementation
of these measures will be 30150 AED.
By spending this amount the company can prevent the outbreak of fire and damage to
buildings and equipment’s. This will help create a positive safety culture, increase in the worker
morale and prevent accidents and increased premium costs and other costs associated which will
result when accidents happen, and the company can also ensure that the social, economic and legal
obligations are met.

2. ELECTRICITY (Observation No. 1, 15)

Electricity is one among the major hazard found in the work place it can cause damage to
property and can impart severe personal injury even fatality with an increase in the financial
liabilities to the company. Working near live and damaged live electric wires can cause severe shock
to the workers. During the inspection I could notice some bad practices with electricity like use of
poorly insulated cables in the welding machine, working with MEWP near overhead power lines.
When a person comes in contact with the electricity, he will get electric shocks with burns at the
point of contact and at the point current flows out of the body. There may also be internal burns to
the organs which falls along the path of the current flowing through the body. This will result in the
employer losing a competent staff from the services and cost of repairing the damaged machinery
and equipment’s. The Employer must take care of the hospital expenses for the worker, sick pay may
needed to be given to the worker and also there will be an increased insurance premium cost. To
add to these woes the company may have to deal with civil claims, enforcement actions and loss of
customer good will and employee trust.
As per the article 10 of ILO C155 and R164 it is the responsibility of the employer to provide safe
place of work, safe plant and equipment. The observed risks are in violation to the laws of the
international labour organisation. UAE has adopted the British Standard BS 7671 (IEE Wiring
Regulations, UK) Regulations which came into force on 1 March 2014 also deals with electrical wiring
related issues. The UAE Electricity Wiring Regulations (Third Edition) provide requirements for the
installation and operation of safe electricity wiring systems in customers’ premises.
Recommendations for maintaining the legal standards includes the replacement of damaged
wires, provision of sign boards and goal posts to the area where overhead power lines exist, provision
of nonconductive PPE like rubber gloves etc, nonconductive tools and providing training and
supervision. The estimated costs for implementing these control measures will be 6250 AED.
By implementing these control measures accident rates will be reduced and we can achieve a
good production rate, we can increase the morale of the work force. So, by taking these actions the
company can achieve a good reputation and customer good will by completing the supply demands
on time. This will result in increase in the production rate, less downtime which will result in
increased profitability and the company can also ensure that the social, economic and legal
obligations are met.

3. CONFINED SPACE (Observation No. 25)

This is another important area in the company which needs proper attention. I found a group
of workers working inside a confined space carrying out welding activity on carbon steel pipeline. They
were carrying out the activity without conducting gas tests, lack of personal monitor was observed.
This pose a serious danger as there is no means to find out if there is any presence of hazardous gas
mixture inside the confined space which can cause fire or explosion leading to serious burn injury or
even fatality. Workers were not aware of the safety precaution to be taken while working inside a
confined space. The lack of competent worker doing confined space activity pose serious risk as they
will put theirs as well as other co-worker’s life in danger. There was no rescue equipment available at
the location and neither was the permit accompanied with a rescue plan. This pose a serious risk to life
as valuable time will be lost in trying to get the rescue equipment’s on site and this could mean the
difference between the patient surviving or dying. This will negatively affect the health and safety
culture of the organization.
The management of the organization should take important decisions to reduce the risk
related to the confined space work. They must ensure that the only people who are trained in
confined space activity can work inside the confined space policy is properly followed. They must
develop a safe system of work or special written procedure (ILO-OSH-2001) for working inside a
confined space and maintain proper data entry of the workers entering and exiting the confined
space. Strictly follow permit work system for confined space work activity and attach the rescue
plan. Provide adequate supervision. Provide work breaks and welfare facility to the workers by
providing clean drinking water and temporarily rest room.
As per the article 10 of ILO R164 and C155, UAE Federal Law N.O:8 of 1984(UAE Offshore
law), UAE Labour Law (UAE Federal Law N.O: 8 of 1980) it is the responsibility of the employer to
provide safe place of work, safe plant and equipment. The UAE Federal Law N.O:8 of 1980 article
145 deals with compensation for occupational hazards.
Recommendations for maintaining the legal standards includes providing all eligible
candidates with the confined space entry training. Provide emergency rescue equipment
arrangement near the confined space area. Provide barricades and sign boards for the confined
space. The Total cost associated is about 6700 AED.
By putting in these measures and actions the management will earn trust of its employees and
people will help generate a positive safety culture. Company can avoid compensation Claims,
reputation of the company will be increase, and the company can also ensure that the social,
economic and legal obligations are met.

4. LIFTING (Observation No. 5, 13)
Hazards associated with the Lifting activity was found to be one of the major hazards
prevailing in the company. It was observed that unsafe lifting activity and improper lifting procedure
and safety norms were being followed. Improper lifting procedure can create imbalance in the centre
of gravity of the load and the load will become uncontrollable which might swing out of control and hit
other structure, the lifting sling might get snapped and the load can fall on the crane or people working
nearby. This could lead to fatal injury, loss of equipment’s and Vehicle etc which will affect the
company financially and could hamper the company’s image due to the fines and enforcement actions
that will follow as a consequence of the accident. It will also lead to delay in carrying out the intended
work as a result of the investigation that will be conducted, and corrections needs to be implemented,
risk assessment also needs to be updated accordingly. I observed that in the drilling workshop workers
were lifting heavy drilling pipes manually and placing it on the low bed truck even though HIAB Truck
(which is used for lifting and placing the load on to the truck) was available as there was shortage of
trained operators who could operate the lifting equipment in the process the workers were subjecting
themselves to the risk of getting Musculoskeletal disorders, WRULD, Hernia etc. This will result in
reduced work efficiency and output from the work force and might also lead to absenteeism due to
the need for taking rest and getting treated in the hospital. The company will have to bear the medical
expenses and pay sick pay also for the same and could also be subjected to claims for compensation.
As per the article 10 of ILO R164 and C155 and UAE Labour Law (Federal Law N.O: 8 of 1980)
it is the responsibility of the employer to provide safe place of work, safe plant and equipment. The
observed risks are in violation to the laws of the international labour organisation. Lifting Equipment
is subject to the Lifting Operations and Lifting Equipment Regulations (LOLER) 1998. Lifting
Equipment is also subject to the Provision and Use of Work Equipment Regulations (PUWER) 1998.
DM- "Code of Construction Safety Practice". DM Technical Guidelines 41 pursuant to Local Order 61/
1991, “Examination and certification of crane, hoists, lifts and other lifting appliances”. Violation of
these laws will lead to severe penalty, enforcement action and imprisonment of personnel.
Recommendations for maintaining the legal standards includes providing more trained lifting
operators, provide training to workers, provision of sign boards and goal posts to the area where
overhead power lines exist. Provide a lifting supervisor. The estimated costs for implementing these
control measures will be 25500 AED.
By implementing these control measures will help to improve health and safety performance
standards and will reduce the chances of accidents. Thereby the company can save more man hours
and services can be done on time. This will result in increase in the production rate, less downtime
which will result in increased profitability and the company can also ensure that the social, economic
and legal obligations are met.

5. Biological (Observation No.11)

This is another important area where the company needs to give more importance. I observed
that in the senior mess hall foul smell was coming from underneath the mess hall due to stagnant
water accumulating as the drain pipe was leaking contaminated water on to the ground and excess
water had accumulated underground. This will attract rodents and there by a risk of Legionella and
Leptospira bacteria arising from the stagnated water in the drainage which diseases like legionaries’
disease and leptospirosis diseases. It also serves as a good breeding area for the mosquitoes and
thereby increasing the chances for health problems like chicken guinea. If a person with open
wounds or cut comes in contact the waste water Leptospira bacteria will enter through the wound
causing leptospirosis disease, Legionella enters the body through inhalation of the contaminated
water droplets. It also causes work related ill-health and allergies if the working condition is
unhygienic, it will affect productivity and will damage the company goodwill.
The ILO convention C 164 paragraph 1a, UAE Federal Law N.O (11) of 2006, UAE Federal Law
N.O (03) of 1993, HAAD (Abu Dhabi health authority) deals with protection of environment control
and states that an employer should provide environment free from risks of any foreseeable diseases.
The control measures that needs to be implemented immediately are bleach the water and assign a
plumber to fix the leak in the drainage also the housekeeping quality must be improved. Remove/
Displace the stagnant water completely from below the mess hall area.
The recommendations for maintaining the legal standards includes assigning a plumber to fix
the leak in the drainage and provide vaccination to all employees eating in senior mess. The cost for
implementing the control measure is 2500 AED. On implementing the control measures mentioned
above the organization will get safe environment, disease free employees. The organization can
avoid losing manhours to disease and save money involved in treating sick patients and providing
them sick leave payment and the company can also ensure that the social, economic and legal
obligations are met.

CONCLUSIONS

I am extremely grateful to ADNOC Offshore Company for the allowing me to conduct this
inspection and I would also like to thank all the contractor employees and ADNOC employees for
their cooperation. The overall health and safety performance of this organisation is appreciable. The
organization considers promoting good health and safety in the work place as its prime duty. Overall,
I feel that the company has a good safety culture and the workers are also by and large dedicatedly
following the safe work system. The company has good induction program for new employees and
visitors and that too in 3 different language to carter the needs of the large work force. During the
inspection I observed good practice of safe behaviour and coordination by the workers, daily
mandatory tool box meeting was dedicatedly attended by the workers. The company organises
motivational programs, Safety plays and routinely conducts mock drills for emergency preparedness.
The recommendations mentioned in this report should be achieved within the realistic timescale
provided.

There were also some major issues noted during the time of inspection which requires
immediate attention of the top management to avoid accidents and to improve production rate and
to meet the legal standards.

Fire safety is not that effective in the workplace. Hot works are being carrying out hot works
close to oil well without doing gas tests, carrying out welding activity in hazardous area with
minimum or no personal gas monitors with them, obstructed fire exit route was seen in the
Substation, Lack of fire extinguisher in required area. These activities increase the chance of fire in
the workplace and difficulty in carrying out evacuation process. The people may get seriously injured
or died because of the fire and smoke.

It was observed that the workers are prone to Electrical hazards. The wires of welding
machine were found to be faulty, insulations of the cable were broken and was covered with cello
tape instead of proper insulation tape, the workers were carrying out electrical work without using
proper PPE thereby increasing the risk of electrical shock. The workers may get exposed to electricity
and may get electric shock or electric burns. A worker was doing touch-up painting using MEWP
which was placed very near to the live overhead line. The worker may get severe electric shock and
electric burns if he comes in contact with overhead line and this may lead to fatality.

Worker awareness towards working inside the Confined space was found to be wanting. It was
found that a group of workers were doing unsafe act by working inside a confined space carrying out

It is the moral responsibility of an employer to give safe place of work, safe plant and equipment
and safe system of work. The international bodies like International Labour Organisation and laws of
national governments of individual countries puts a responsibility on the employer to maintain good
standards of Health and Safety. Failure to comply with these legal standards may lead to
enforcement action and may also result in prosecution and imprisonment. The company will also
suffer direct and indirect costs that will arise from accident. Hence by maintaining a good health and
safety standards, the management could achieve significant profit, customers good will and its
employees trust.
