AFFIDAVIT OF LOSS
I, JASPER DELA CRUZ of legal age, Filipino, married, and a resident of Polomolok
South Cotabato, after having been sworn to in accordance with law, do hereby depose and
state, THAT ---

1. I am employed as Engineering Clerk at Tuna Exporters, ECA Building, Door A21,

National Highway, General Santos City;

2. In my everyday way to and from the office, I wear my Company Identification Card
to be identified as an employee therefrom;

3. Sometime on September 2018, while on my way to work, I noticed that I misplaced

my Company Identification Card and is nowhere to be found ;

4. Diligent and earnest efforts to recover the said Company Identification Card were
made, but all did not yield a positive result, thus the said Company Identification
Card is considered LOST for all legal intents and purposes;

I am executing this AFFIDAVIT OF LOSS to attest to the truthfulness of the foregoing

facts, in support for my RESIGNATION and for whatever legal purpose/s it may serve.

Affiant further sayeth naught.
