Instructions for filling the Application form:

1. The Application form must be filled in BLOCK letters.

2. * Marked fields are mandatory.
3. Following documents to be sent via post at the ISST Distance Education Office after completing online registration:
       (a) Photo Copy of latest qualification. (Check Qualification required for the course you wish to enroll for)
       (b) If mode of Payment is DD: Course Fee DD payable at Pune in favor of VMAX SPORTS & FITNESS.
       (c) Two passport size photographs.
4. After online registration, the required documents must be sent via post/courier at the ISST Distance Education Office address mentioned below within 7 (seven)
working days, failing to which the admission will not be confirmed.
