WAIVER FORM FOR TOUR GUIDING INTERVIEW

allowing my son/daughter ____________________________________ (name of student), to actively
participate and conduct an interview of a tour guiding company/tour guide as a mandated by the circular
requirements of the program Bachelor of Science in International Travel Management Major in Travel and
Tour Operations.

This waiver is recognition and affirmation that the Our Lady of Fatima University – Valenzuela
City legally prescribes to the circular program of the Commission on Higher Education and that therefore
the University is not liable for any untoward and unforeseen incident/illness that may occur as a result of
the Tour Guiding Company Interview, whether on-site or coming or going to the tour guiding company.
