INTRODUCTION
The family is the basic unit of the society and in contrast to community health nursing it
is considered as a unit of care. Consequently, it is the primary recipient of the nursing effort,
which is contributory to the development and progress of the community through active
involvement and self-responsibilities of each constituent of a father mother and children. The
impression or status of each family will always affect the status of the community as a whole.
Community health nursing is a response to the needs of the people. It does not focus on a
particular class or family alone. It is comprehensive and general in approach, the community as
a whole. Promotion and preservation of the health of different clients such as individual, family,
The community is a group of people sharing geographic boundaries and / or values and
interests. With this a nurse exposed in the community learns how to interact and adopt to
different kinds of people because good communication brings understanding between a family
and a nurse which is vital to meet the needs through building rapport. It is in the family where
a member develops his health values, beliefs, and practices. The family is a major influence in
the health behaviors of an individual. Thus, it is important that families in the community are

community through its basic structure – the family. It is a tool in determining the health status
of a family through assessment and critical inspection. Through this, health related problems
are identified, thus giving the student nurses a hint on where to act and how to intervene. It is
also a means towards improving the health of the community people, making them more
productive. To come up with a family case load gives a sense of fulfillment to the student
nurses as they are given the opportunity to share their skills, knowledge and time to alleviate
and uplift the living condition of a family. The family that was chosen by us is a picture of the
majority of the family here in our country: a family living in a poor environmental condition
without enough resources, lacks knowledge on vital health information and exercises other
socio-economic related with the problem. Though tiring as it is, reaching out to this family and
mingling with them makes the student nurse feel the sense of fulfillment.
OBJECTIVES
GENERAL OBJECTIVE
The family case load is designed for us student nurses and other health care
providers to widen our knowledge on Community Health Nursing, enhance our skills to
the delivery of basic health care services, and develop our right attitude in dealing with
SPECIFIC OBJECTIVES
To develop the right attitude in dealing with the different members of the
family.
INITIAL DATABASE
The head of the family is Mr. DS. He is 45 y/o and was born on September 07, 1975. He works as a
Construction Worker. His wife is Mrs. ES who is 40 y/o, and was born on October 05, 1980. She works as
a part time laundry washer. The couple has six children namely: DS1, DS2, DS3, DS4, DS5, and RS with
ages 18, 12, 11, 5, 3, 1 y/o and 3 months respectively. The family lives in Barangay San Esteban, Iguig,
Cagayan. Both the husband and wife decides for the family. The type of the family structure they have is
a nuclear. So far, the family has no conflict with each members. They use Ilocano dialect in their
household and Tagalog when they are communicating outside.
Mr. DS is a construction worker that works Monday through Saturday and makes P300/day. Mrs. ES
is a part time laundry woman that only works twice a week and makes P300 per load. The monthly
family gross income is Ten Thousand Eight Hundred Pesos (Php 9, 200) and they get additional Four
Thousand Eight Hundred (P4,800) quarterly which is equivalent to One Thousand Six Hundred (P1,600)
monthly from 4P’s. Both of them manages their financial budget whereas the family is able to meet their
basic needs such as foods, shelter, and clothing. Mr. DS is a Highschool graduate with diploma and Mrs.
ES is a elementary graduate. Their first child, DS1 is currently a 12th grade student and the second child,
DS2 is a 7th grade student at Iguig National High School. The third child DS3, is currently in 6th grade at
San Esteban Elementary School. The fourth child, DS4 is in kindergarten. The fifth and the sixth child,
DS5 and RS are not yet studying and stay at home with Mrs. ES. The family’s religion is Catholic and they
usually visit their church and attends prayer meetings. The family also participates in the activities of
their community.
The family lives in a small house with steel roofing and wood walls with an area of 42 square meters.
The house has no room and no windows, it has two beds where they all sleep together. Also, kitchen is
located inside with a dining table and and a small living room with a TV and radio. The household’s
drinking water is stored in a plastic jar with a faucet and is from a pump well located at their neighbor’s
house with a distance of 26.52 m to the house. They have their own toilet which is a pit-privy type and
unsanitary, it is located about 5 meters from their water source which is different from their source for
drinking water, also their bathroom is separated from the toilet which is 3 meters away. The family also
does not practice proper waste segregation and disposal, also they don’t have a drainage system.
Transportation facilities are readily available from their place.
Mrs. ES has a dry cough for 5 days and her children have also present illnesses. The fourth child DS4
has cough and runny nose. The fifth child DS5 has been having a fever for 3 days . The sixth child DS6 has
been having a fever at night. Hence there has been spread of flu like symptoms in their household. The
mother practices the use of herbal medicines like boiled Lagundi plant for cough and also uses over-the-
counter drugs like paracetamol for fever. She also stated that when the illness of the family member is
severe, they seek medical assistance in their nearby Barangay Health Center. The family eat vegetables
on a daily basis from their small vegetable garden and when their finances permits to do so, they
incorporate meat in their meals
The family sleeps 6 hours except for the toddlers DS5 and RS who sleep around 8 pm and
wakes up at 6 am. The father works as a construction worker that gives him a physical exercise
everyday while the mother do the household chores. As a form of relaxation, the father drinks
alcohol occasionally to relieve his stress while for the mother and the children they bond together,
read magazines or watch entertaining show in the television. During the night, the family uses bed
nets to protect them from mosquito bites. The family are into vegetables which is good since they
have their own little vegetable garden to maintain good health. Immunization were given to them
except for the sixth child who will be getting his last dose of measles on Oct. 16, 2019.sss

Mrs. ES has a dry cough for 5 days and her children have also present illnesses. The fourth child DS4

mother practices the use of herbal medicines like boiled Lagundi plant for cough and also uses over-the-
counter drugs like paracetamol for fever. She also stated that when the illness of the family member is
