Dear Sir/Madam,

This is to inform you that my daughter Irish Andrea G. Esperanzate of Grade 11-STEM will
be having an operation next week. A cyst is seen on the upper part of her uterus which causes her
abnormal menstruation cycle and frequent urination and this is urgently needed to remove.

In connection to this, I would like to excuse her tomorrow in your class because she is appointed
to have a check-up at Irosin District Hospital and an assessment as per preparation for her operation. She
will attend your class as soon as possible.
