Contract, you shall be evaluated by the policy of this company, which will determine your qualification or disqualification.
Poor or unsatisfactory performance renders this contract invalid as it serves as a strong ground for the termination of
your services to the company.

Upon acceptance and acknowledgement of this letter of appointment signified by your own signature, it is clearly
understood that you bind yourself with the following terms and conditions of the company amongst others.

1. You agree to willfully abide the company rules and regulations as stated in the company policy manual without
reservation.
2. You agree to obey and follow the in-house company policies, without objections. Company Policy will be
provided to you separately and likewise be oriented.
3. That the employee recognizes and adheres with all company policies during the period of employment in this
contract.
