AFFIDAVIT

I, ANTONIO E. MARAVILLA, of legal age, Filipino and a resident of Bacolod City,

Philippines, after subscribing to an oath in accordance with law, do hereby depose and state that:

1.) Previously, I have executed a Special Power of Attorney appointing one Romanito M.
Solinap to be my true and lawful attorney-in-fact for purposes of executing and filing an
affidavit to loss to declare the fact of loss of the original Owner’s Duplicate of Transfer
Certificate of Title No. T-268443 covering my parcel of land known as Lot 9-B-1-B-3
situated in Brgy. Estefania, Bacolod City.

2.) Pursuant to the authority I gave in favor of Romanito M. Solinap, an Affidavit of Loss
was executed by him and was caused to be registered before the Office of Register of
Deeds of Bacolod City and annotated was, along with the Special Power of Attorney
dated April 2, 2012, annotated as Entry No. 2012003161 dated May 3, 2012.

3.) Since the time of the registration of the said Special Power of Attorney and Affidavit of
Loss, no such petition was ever filed for the purpose of securing the issuance of a second
Owner’s Copy of Transfer Certificate of Title No. T-268443 and for that matter, I have
decided to revoke the authority I previously issued in favor of Romanito M. Solinap and
execute a new Special Power of Attorney appointing Edgardo Severino as my new
attorney-in-fact for purposes of causing the issuance of the said second Owner’s Copy of
Transfer Certificate of Title No. T-268443.

4.) As such, I hereby declared under oath that I am revoking the authority which I previously
granted and/or issued in favor of Romanito M. Solinap and likewise declare under oath
the fact of loss of the original Owner’s Duplicate of Transfer Certificate of Title No. T-
268443.

5.) Said Owner’s Duplicate of Transfer Certificate of Title No. T-268443 was not conveyed
as security for any loan and was lost due to inadvertence from among my other equally
important documents.

6.) Despite having exercised diligent effort to try and locate the said Owner’s Duplicate of
Transfer Certificate of Title No. T-268443, still, the same could not be found and is not
deemed lost and beyond recovery.

7.) I am executing this affidavit to attest to the truth of the foregoing and for whatever legal
purposes this may serve.
 IN WITNESS WHEREOF, I have hereunto affixed my signature this ____ day of
