AFFIDAVIT OF CHANGE BODY

I, ANGEL G. BERMUDEZ, of legal age, Filipino and a resident of

Sto. Niño 3rd, San Jose City, Nueva Ecija after having been duly sworn to
in accordance with the law do hereby depose and state that;

1. I am the registered of a motorcycle hereunder described as

follows:

SERIES : CT100S
BODY TYPE : MC
ENGINE NO. : DUZWEB26946
SERIAL CHASSIS NO : MD2A18AZ9EWB87110

2. I am intending to change the body of the aforementioned

motorcycle from motorcycle without sidecar to motorcycle with
sidecar.

3. The sidecar is acquired from FADER EN SON SIDECAR, Abar 1st,

San Jose City on March 10, 2016 in the amount of PHP 10,
000.00.

4. I am executing this affidavit for the purpose of attesting to the

truth of the facts contained;
