AFFIDAVIT

I, FLORENTINA CLEOFAS y CAPARAS, Filipino married

to CARLITO CLEOFAS, of legal age, with address at Purok 5
Bagong Tuklas St., Santor, Malolos City, after having been
duly sworn to in accordance with law hereby depose and say:

1) The Aid Information Sheet issued on July 10, 2017 at

Provincial Capitol indicates my address as Brgy.
Anilao, Malolos City, Bulacan.

2) On the other hand, on my Voter’s ID issued

sometime in year 2000, issued by COMELEC, my
address was indicated as Purok 5 Bagong Tuklas St.,
Santor, Malolos City.

3) Both names/documents pertain to one and the same

person.

4) I am executing this affidavit to attest to the truth

and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my
