The forms shall be available on the NDMC Website also. 2. Fee Structure Within 21 days of the occurrence of Birth/Death- No Fee for registration of Birth/Death. First free copy is provided by the Hospital. After 21 days to 30 days of the occurrence of Birth/Death - Rs. 7/- As late fee After 30 days to within 1 year of the occurrence of Birth - Rs. 10/- As late fee Beyond One year of the occurrence of Birth/Death - SDM Order from local area + Rs. 15 As late fee Name Registration in Birth Certificate fee Rs. 10/- Birth Certificate fee Rs. 20/- for one copy Death Certificate Rs. 10/-

Hand written application to Registrar birth having subject name No fee inclusion of child by the concerned child parent and to be submitted at designated birth centers.
2 After One Year:
i) Hand written application to Registrar birth having subject name inclusion of child by the concerned child parent and to be submitted at International Inoculation Centre (HQ) Birth Centre, Mandir Marg. Rs.10/- As late fee + Affidavit ii) Affidavit stamp paper on Rs.10/- duly attested by notary public having subject name inclusion of child with supporting documents like Passport, Ration Card, Voter Identity Card, 10th class certificate etc. CORRECTION IN BIRTH /DEATH CERTIFICATE
A. INSTITUTIONAL BIRTH/DEATH A correction letter is required from Medical
Record Officer of concerned hospital/institution where birth/death event occurred along with supporting documents for events which are less than 10 yr old
B. NON-INSTITUTIONAL i) Affidavit stamp paper on Rs.10/- duly
BIRTH/DEATH attested by SDM/Notary public having subject correction of birth/death events For Birth & Death event > 10 yr old ii) Supporting document like Passport, Ration Card, Voter Identity Card, 10th class certificate, cremation receipt etc.
