1. Application procedure Birth/Death application form duly filled by

head of the family/Relative etc which are
available at International Inoculation Center
(HQ) Mandir Marg/ Palika Suvidha Kendra/
Other Concerned Areas Birth and Death
Centers.
The forms shall be available on the NDMC
Website also.
2. Fee Structure  Within 21 days of the occurrence of
Birth/Death- No Fee for registration of
Birth/Death. First free copy is
provided by the Hospital.
 After 21 days to 30 days of the
occurrence of Birth/Death - Rs. 7/- As
late fee
 After 30 days to within 1 year of the
occurrence of Birth - Rs. 10/- As late
fee
 Beyond One year of the occurrence of
Birth/Death - SDM Order from local
area + Rs. 15 As late fee
 Name Registration in Birth Certificate
fee Rs. 10/-
 Birth Certificate fee Rs. 20/- for one
copy
 Death Certificate Rs. 10/-

 Search fee Rs. 7/- for old records

iv) SBS Place (for current year certificate for

Gole Market and nearby NDMC area,
L.H.M.C Hospital & R.M.L Hospital,New

v) Charak Palika Hospital, MotiBagh,New

Delhi. (For Current Home & Hospital Data,
Motibagh, Sardar Patel, North & South
Anveue, Ashoka Hotal, Netaji Nagar etc.)

vi) Maternity Hospital, Lodhi Road,New

Delhi. (For current certificate(PMH/Neaby
home data) Zone -8 SI office

 Entry of Child Name in Birth Record

1 Within One Year:

Hand written application to Registrar birth having subject name
No fee
inclusion of child by the concerned child parent and to be
submitted at designated birth centers.

2 After One Year:

i) Hand written application to Registrar birth having subject name
inclusion of child by the concerned child parent and to be
submitted at International Inoculation Centre (HQ) Birth Centre,
Mandir Marg. Rs.10/- As late fee +
Affidavit
ii) Affidavit stamp paper on Rs.10/- duly attested by notary public
having subject name inclusion of child with supporting documents
like Passport, Ration Card, Voter Identity Card, 10th class
certificate etc.
 CORRECTION IN BIRTH /DEATH CERTIFICATE

A. INSTITUTIONAL BIRTH/DEATH A correction letter is required from Medical

Record Officer of concerned
hospital/institution where birth/death event
occurred along with supporting documents
for events which are less than 10 yr old

B. NON-INSTITUTIONAL i) Affidavit stamp paper on Rs.10/- duly

BIRTH/DEATH
For Birth & Death event > 10 yr old
attested by SDM/Notary public having
subject correction of birth/death events
ii) Supporting document like Passport,
Ration Card, Voter Identity Card, 10th class
certificate, cremation receipt etc.
