Aliweng-weng Supa, my mother, to act on my behalf in all manners relating to all
dealings with the Professional Regulation Commission (PRC) for RENEWAL of my
PRC License, including signing of all documents relating to these matters. Any and
all acts carried out by Mrs. Myrnalyn Aliweng-weng Supa, on my behalf shall have
the same effect as acts of my own. As I won’t be able to claim it personally as I am
on duty at CIDG, Camp BGEN Rafael T Crame, Quezon City. Attached herein, is a
photocopy of my Identification Card.
