We are pleased to certify and appoint you as NDE Level III for Besmindo started from November 2nd,
2019.
This appointement is based on:

1. Your valid ASNT Level III Certificate No. 228051 in Electromagnetic Testing, Liquid Penetrant Testing,
Magnetic Flux Leakage Testing, Magnetic Particle Testing and Ultrasonic Testing.

2. Review of your education, training, qualification, expriences and certification records.

Your appointment will be valid for a period of 5 ( five ) years from the date this appointment. This
appointment can be extended for further mutually agreed period based on your satisfactory
performance and provided that you maintain the validity of endorsement on your ASNT certificate in all
of the above NDE methods.

You have been exempted from the specific Examination requirements as per clause ASME BPV Sect. V,
ASME BPV Sect. VIII Div.1, AWS D1.1, ASTM E165 – 18, DS 1 Vol 3 & 4 based on our review of the
documentary evidence of experience including your experience in the preparation of procedures to
ASME codes and evaluation of test results.

You will be responsible in :

 Establishing, maintaining and approving our company’s NDE Personnel Qualification and
Certification procedure.
 Establishing and Approving ET, PT, MFL, MT and advanced UT Procedures.
 Demonstrating, qualifying and certifying NDE procedures as per the applicable technical
references (codes and standards) our customer’s requirements.
 Resolve NDE related issues on behalf of our company.
 Perform as “Outside Training Agency” in providing NDE training and qualification for our
employee(s).
