    Camarines Norte registered in the name of GLESIE F. CABALLO with TIN No. 182-902-062-000.

    Attached herewith are the tax payments for Annual Income Tax Return 2018, Quarterly Income Tax,
    Quarterly Percentage Tax, Annual Registration Fee and other documents necessary for the issuance of the
    said tax clearance.
