MEMORANDUM TO BE SUBSCRIBED ON THE WRIT

This writ may not be served more than six (6) calendar months after the above date unless
renewed by order of court.

The defendant may appear hereto by entering an appearance either personally may, if
he desires, enter his appearance by post, and the appropriate forms may be obtained by sending
a Postal Order for RM XXX with an addressed envelope to the Registrar, High Court of Kuala
Lumpur at Kuala Lumpur Federal Territory.

obtain the Grant of Probate a forgery and was null and void;
(ii) An Order for Revocation of the Grant of Probate granted in favour of the
Defendant in respect of the estate of the Deceased;
(iii) Costs of this action;
(iv) Such further and/ or other relief that this Honourable Court deems fit and proper.

And RM XXX (or such sum as may be allowed on taxation for costs, and also, if the plaintiff
obtains an order for substituted service, the further sum of RM XXXX (or such sum as may
be allowed on taxation)
