NSED Post Activity Evaluation Report Template

I. Introduction (that includes but not limited the office / agencies disaster risk profile and mandates)

In compliance to the Division Memorandum No. 168 s. 2019 Conduct of the 2ND Quarterly
Nationwide Simultaneous Earthquake Drill (NSED) Nationwide, the quarterly conduct of NSED pursuant
to Rule 6, section 7 of the Implementing Rules and Regulations of RA 10121, Catablan Integrated School
conducted earthquake drill in the school campus. Sir Tedie A De Vera, the School Principal, and Ma’am
Lourdes R. Pabito, the Head Teacher led the activity with Ma’am Rosalina A. Taluban, SDRRMC
Coordinator in Elementary and Sir Mark Jay Q. Bongolan, SDRRMC Coordinator in Secondary also with
the teachers and respective classes. The drill was carried out simultaneously and smoothly. All grade
levels from Kindergarten, Elementary, Junior and Senior High School students occupied the designated
area for them, based on the school evacuation plan.

II. Background (that includes time table of activities such as series of activities, efforts and preparations
before the actual drill)

Catablan Integrated School conducted an earthquake drill in the school campus last June 20,
2019 at exactly 2 o’clock in the afternoon. The said drill started with the buzzer sounded for one minute
as a signal that an earthquake is currently happening. Everybody in their respective classes performed
the Duck Cover and Hold Technique to protect themselves from the tendency of falling objects that may
harm them. After that, the advisers and the designated SDRRMC with the assistance of the School
Principal Mr. Tedie A. De Vera led the pupils/students in heading to their designated evacuation area
with a free-hustle movement to avoid panic and stampede. After they reached the area they were
grouped together according to the class where they belong. Then, the classroom teachers and the
assigned students in each class performed a headcount to make sure that all the pupils are accounted
for. The SDRRMC coordinators announced the termination of the drill.

IV. Comments Observation (by the evaluators)

The drill was done successfully. Students, teachers and other stakeholders actively
participated in the drill.

V. Analysis

The drill intensified the students’ awareness regarding disaster risk reduction whenever
they are in time of incidences. On the other hand, this helped a lot the teachers and the school in
managing emergencies especially, the earthquake occurrences. The drill was successfully accomplished
by the learners with the proper encouragement of all teachers.
VI. Recommendations

Teachers should be the last one to go out of the classroom to ensure that all occupants of
the room have evacuated.

Pupils and students must stay calm in case there is an occurrence of earthquake.

A buddy- buddy system should be applied by the participants.

Earthquake drill should be done quarterly to ensure that the students, teachers and other
stakeholders are ready in case there is an earthquake.
