CIVIL OR CRIMINAL CASE

I, JOHN DAVE L. DIONSIO, Filipino, of legal age, with office address at M.

Villarica Rd., Sta. Rosa I, Marilao, Bulacan, after having been duly sworn to in
accordance with law, depose and say that:

1. I am currently employed at Marilao, Bulacan;

2. I have no pending case, criminal or civil, before any judicial

authorities or agency of government;

3. I am neither an advocate nor mentor of any unlawful or subversive organization

that advocated the overthrow of the government or agency in the government;

4. I was never accused of any offense or crime involving moral

turpitude, neither one of the same nature or a pending case of anti-
graft law (RA 3019) against my person is pending before any
prosecution office and judicial authorities elsewhere in the
Philippines;

5. In view of the foregoing I am executing this Affidavit to attest the

truth of the foregoing facts and for whatever legal purpose it may
serve.
