Project) of the Department of Health HRH 2020 to be assigned back at Rural Health Unit of Libjo,
Province of Dinagat Islands.

I am JOGIELYN LABORADA VARGAS, 30 years of age, single, soloparent and a resident of San
Vicente, Tubajon, Dinagat Islands, graduated Bachelor of Science in Nursing at Surigao Education
Center, Surigao City year 2010, passed the July 2011 Nursing Licensure Exam and become a
licensed/registered nurse with PRC ID 0730957 renewed last Dec. 14, 2018.

I was one of your NDP Nurses hired 2014 up to 2018 and was rehired last year, July 2019 as one
of your Public Health Associates. My 6 years of working experience as a community health nurse
and half a year of experience as PHA has taught me enough knowledge and skills that would
made me an effective health worker in the grassroots of DOH, perform best with the duties of
my desired position.
