BPPL CARDINAL SAFETY RULES

BPPL Cardinal Safety Rules are established at Byco Plant and off sites facilities to provide clear
guiding rules to all Byco employees and Contractors. They are required to follow these rules
when performing certain high risk activities. Deliberate Failure to comply with BPPL Cardinal
Safety Rules may result in immediate disciplinary action. Severity of these disciplinary actions
may be up to the termination in case of BPPL staff, or no longer being permitted to work for
BPPL site in case of contractors.

1) Work with a valid permit when one is required.

a. All work to be carried out in the Refinery’s area of responsibility must be covered by
a work permit. Type of permits, authorized persons, validity and permit attachments
are found in the BPPL Permit to work procedures.

2) Enter a confined space only after receiving a valid entry permit and permission by Stand
by man. Stand by man shall allow entry in confined space only with a valid entry permit.

a. Make sure you have received valid confined space entry permit before you enter
confined space. Precautions mentioned in Confined space entry permit must be
clearly understood and disseminated to all team members.

3) Before conducting any job, verify relevant equipment has been isolated, de-energized and
lockout and tagged out when required; and wear the life protecting equipment specified
for the task.

a. Before allowing work to start, make sure that any kind of energy (electrical,
hydraulic, pneumatic, steam, water, chemical and mechanical) has been positively
disconnected. If type of energy is electrical, always test by actuating the switch
button. Wear all PPEs stated on the permit and if you are unable to fulfill the permit
conditions, do not start work.

4) Do not remove a lock out/ tag out device without receiving proper authorization.

a. Obtain permit to remove locks and to energize the equipment; check and make sure
permit for the work for which caused the isolation has been signed off and handover
procedure completed. Remember to replace lock out tags should the situation
require the equipment to remain de-energized

5) Conduct a gas test when required by the work permit.


a. Check that the required gas test has been done, recorded and signed on permit
before commencing work.

6) Do not by-pass critical safety equipment without receiving proper authorization.

a. Never by-pass any critical safety equipment like shut down relief valve, Level switch
or ESD etc without receiving approval from management. Do not create
unauthorized process/ plant change. Make sure you are not obstructing access to
fire alarms, emergency equipment like safety showers, fire extinguishers etc.

7) Do not leave the site without handing over charge to next shift reliever.

a. You will not leave the Complex / Plant / Unit / Installation without handing over. If
next shift is late or not coming for any reason, you will perform your duty as routine
until next shift reliever take handing over from you.

8) Report all “On the job” unsafe act/ condition, near miss and incident.

a. Do not hide or ignore unsafe act/ condition, near miss or incident. Inform it to your
supervisor immediately

9) Properly protect yourself against a fall when working at height.

a. The required fall protection is a full body harness and lanyard with shock absorber.
Safety belts are not allowed; Lanyard without shock absorber are not allowed except
where it is used as fall restraint (not fall arrest) system.

10) Do not walk or work under a suspended load.

a. Always look for overhead suspended loads or equipment’s while walking or working
in site area.

11) Do not smoke in plant area except designated smoking areas.

a. Smoking is not allowed when you enter plant or offsite facility’ fence/ wall. Only
smoke in designated areas. Always ensure to properly extinguish the cigarette flame
b. after you finish your cigarette.
12) Do not bring mobile phone, match box, lighters and cameras inside the fence/ wall of
plant/ offsite facility.

a. Always submit your cell phones, match box, lighters and cameras at the time office/
security check post before you enter the complex / plant / installation / offsite
facility.

13) Bringing arms, ammunition and explosive material inside plant/ offsite facilities is strictly
prohibited.

a. Any arms, ammunition or explosive material must not be brought into the complex /
plant / installation / offsite facility. If you are required to carry these things for legal
purpose, you will submit them at main security gate.

14) Do not remove any guards installed on equipment.

a. Never remove or by-pass safety guards on moving machinery. Ensure all guards are
re-installed properly after maintenance is complete.
