CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This contract and entered into by and between EVELYN G. GARLIT of legal age, married
Filipino and residing at Purok 5 Magsaysay St. San Pedro, Laguna otherwise known as LESSOR.

-AND-

, of legal age, Filipino, Married and presently


residing at Block 4 Lot 32, Melbourne St. Phase 1-A, Villa Olympia Subdivision, san Pedro,
Laguna hereafter called LESSEE.

-WITNESSES-

That the LESSOR hereby lets and leases unto the LESSESS and the latter hereby accepts
the aforementioned unit subject to the following terms and conditions to wit:

1. That the monthly rental is


(P ) which is to be paid to the LESSOR or her authorized representative
on or before the of the month and every month thereafter without needs of
demand and in cash only with receipt (any kind of cheque unacceptable);

2. That upon signing agreement, the LESSEE shall pay upon the LESSOR or her
representative the equivalent of 2 monthly rental (as 1 month advance and 1 month deposit)
to answer for unpaid electricity bills of MERALCO, water dues, association dues, garbage
fees dues and broken, damaged or missing items inside the unit like door knobs, door locks
and screen doors and windows, window pane/glasses, toilet water bowls, basins, kitchen
sinks, plumbing like faucets, built-in cabinets doors, lock, broke/damaged tiles ,
ect…during their stay in the unit if the LESSEE will terminate contract of lease before one
(1) year;

3. That 1 moth deposit and 1 month advance shall never be used by the LESSEE as his/her
monthly rental during the duration of his/her agreement and the after one (1) month for the
restart of his/her lease agreement and that after LESSEE shall pay the monthly rental of
P every day of the month promptly and without needs of
demands and is cash only. That the 1 month deposit can be used as monthly rental on the
last 1 month prior to the LESSEE’s termination of lease.
4. That this contract of lease is for one (1) year period only subject to the renewal upon
agreement of both parties with priorities to the approval and consent of the LESSOR or
through his/her representative.
5. That the LESSEE shall use the unit for RESIDENTIAL PURPOSES ONLY and the
LESSEE and the person who indicated to the information sheet stay only;

6. That the LESSEE shall not make any alteration or modification or additional carpentry
works etc. without prior approval for the LESSOR to avoid misunderstanding. If the
LESSOR agreed to make additional or modification the LESSEE cannot remove once the
LESSEE leave the unit;

7. That the LESSEE shall at all times take good care of the unit as good father of the family.
Please observe the noise and late visitors;

8. Visitor can allow to stay 4 days only.

9. One (1) pet can allow only.

10. That the LESSEE is agreeable to settle/pay first his/her dues as mentioned above (30) days
prior to their vacating/ leaving the unit. And if not paid, the LESSEE will leave the
corresponding amount of the bills or dues settlement;

11. That the LESSEE shall secure first a written clearance also from the LESSOR or his/her
authorized representative and a written clearance also from the ASSOCIATION clearing
them for their dues like MERALCO, water, garbage, association etc. Failure of which shall
bar them from vacating/leaving the premises and LESSOR of his/her representative have
the right to hold some of their properties like appliances to answer to their unpaid bills of
dues;

12. That is case the LESSOR intend to used the unti or shall have/sold dispose the
aforementioned unit during the life of this agreement, the LESSOR, shall notify the
LESSEE one(1) month in advance and the LESSEE shall vacate the unit accordingly;

13. That is case where the LESSEE fails to pay promptly the amount rental payment due
(Please see no. 1 of this agreement), the LESSEE shall only be given a grace period of
three (3) days to pay otherwise it becomes due and demandable, penalty 100 a day until
they not settle the payment.

14. That the LESSOR of his/her representative has the right to inspect and make assessment
of the unit three (3) days prior to vacating/ leaving the LESSEE.

15. That is case violation or infringement of any of the foregoing terms and conditions, the
LESSOR or his/her representative reserves the right to terminate immediately this lease
agreement and the LESSEEs hall vacate the premises immediately or at least one (1) week
after the termination of the lease agreement.
