1. PURPOSE:
This document details site specific organizational responsibilities, actions, reporting requirements
and the resources available to ensure an effective, consistent and timely management of
emergencies that may occur on the FTI operational Plants / Sites.
This includes:

 Details of the procedures used to deal with emergencies affecting the Project / Plant
personnel.
 Roles and responsibilities of FTI HSE Team and Project / Plant Supervisor(s) and Team
Members.
 Details of the external resources available to the Project / Plant Team / Personnel in the
event of an emergency and how these resources will be coordinated.

2. RESPONSIBILITIES

2.1 The HSE Head shall:

 Review and approve the Emergency Response Plan


 Ensure that the plan have covered all necessary aspects of HSE related emergencies at
Project / Plant Site.
 Ensure that all required training, Safety Personal Protective Equipment, and safety
items shall be available at site.
 Ensure the plan is effective and drills are conducted.
 Review all evacuation drill follow-up reports and implement changes as required.

2.2 The HSE Engineer / HSE Officer shall:

 Provide support to the project by maintaining / performing site visits to the entire
project at least on bi-monthly basis.
 Regularly review and be familiar with all applicable directives of Projects / Plant owner
related to safety.
 Develop a Site Safety Management Plan (SMP) for the project to ensure a safe work
environment for all company employees, contractors, visitors and community to ensure
compliance.
 Ensure that all team members at Plant Site shall be trained for handling emergency
situations and / or fire in case of fire incidence.
 Ensure that all incidents are reported immediately
 Ensure injured workers are offered modified duties or transferred to FTI with alternate
worker arrangement.
 Ensure that documentation has been completed.
 Monitor and follow up workers having modified duties on Plant.
 Ensure that all necessary safety boards are placed at Plant.
 Ensure that all required Safety Personal Protective Equipment and Safety Equipment
shall be available at plant.
 Communicate, supervisor and assist the field staff to ensure that all safety
expectations are understood and met.

HEALTH AND SAFETY POLICY

OBJECTIVE
To ensure the best possible protection in health and safety matters of all
the employees of organization and those under its influence in the
neighbourhood and in community at large.

MEANS
To achieve this objective, we shall;
 Provide and maintain safe and healthy working conditions, and
upkeep our installations, equipment, process and operations.
 Enhance awareness, skill and competence of our employee and
contractors to effectively involve them in ensuring safety for
everyone.
 Comply with all statutory and regulatory requirements.

The Health and Safety Policy of the company shall be communicated to


the employees and made available to all interested parties.

4. HSE TRAINING AND FREQUENCY OF TRAINING:
Safety Training will be carried out on Bi-annually basis for each shift. All members of each shift will
participate in the Safety Training.

Training provided by the HSE Head and HSE Engineer will cover following aspects/areas of safety.

1. Safety Training in which HSE Head & HSE Engineer (From FTI) emphasizes the usage of PPE’s at SITE
in a right way.
2. Safety Training regarding Chemical Handling at SITE.
3. Training regarding the Usage of Fire Extinguisher in case of Fire.
4. Training about working at height.
5. First Aid Training.
6. General Training comprising on work ethics at SITE.

5. EMERGENCY ESCAPE
In case of emergency, staff present at site will use emergency exit rout as shown in provided
Emergency Escape Route of specific project / plant site. The emergency exits shall be clear of all
obstructions at all times and shall be checked on a regular schedule. If an evacuation of the Plant
is required, all employees will report to the place of Safe Refuge / Assembly Point.
All employees those are not engaged in controlling the emergency will report to the Safe Refuge
Point / Assembly Point and wait for instructions. The supervisor(s) will take headcount to
account for all employees. Plants Layout diagrams of the project / plant site shall be posted to
show all emergency routes to evacuate the plant.
Supervisor or his designated replacement / alternative shall be identified to direct operations in
the event of an emergency. His duties include the following:

 Evacuation of the facility


 Directing any fire suppression
 Accounting for all employees (Safe Refuge)
 Insuring that all new employees are trained on their duty during an emergency
 Assuring timely first aid support to the affected persons in case of any emergency /
accident.
 Assuring transport of affected persons to the nearest hospital.
 Preparation of Accident Report and submission to HSE Engineer at FTI Head Office.
All staff members of each shift shall be trained to deal with any kind of emergency at SITE and supervisor of
present shift is responsible to guide their sub ordinates that how to handle emergency as per instruction
given in Emergency Action plan.

6. FIRE
Only trained employees are instructed to respond to incipient fires (fires that have just started).
At this stage you can sometimes extinguish these fires with the proper extinguishing agent and
Knowledge of how to use a fire extinguisher. Remember - ALWAYS GIVE THE ALARM FIRST.

6.1. ALARM
Smoke detectors / Fire alarm shall preferably be installed at each plant / project site at
appropriate distance. These detectors shall be periodically checked / inspected and maintained
in operational condition.
In case of fire occurrence if fire alarm is not available or fire incidence has occurred in any area
where alarm is not available, alarm shall be given orally by shouting Fire ….. Fire ….. Aag ……
Aag.
If a fire emergency occurs, the first person that identifies the emergency will give the alarm and
respond as directed by their supervisor. In case of major fire, call on the provided Emergency
Telephone Numbers of project / Plant Site.
A List of emergency contact numbers shall be available with the Project Incharge / HSE Incharge
and shall be posted at all Plant / Project locations.

6.2. FIRE EXTINGUISHERS


Appropriate type(s) of fire extinguishers shall be available at each project / plant, in sufficient
numbers to control fire, if occurs. All fire extinguishers shall be inspected periodically for their
validity and expiry.
All fire extinguishers shall be replaced with freshly filled extinguishers having valid expiry, at
least one week earlier of expiry dates on existing ones.
7. FIRE PREVENTION PLAN

Generally no any Flammable materials are stored at SITE during process. The Plant area shall be
maintained in good house keeping condition in a manner to ensure that no potential fire hazard
shall be present.
Housekeeping in the Plant area is mandatory, and all employees shall be instructed to keep walk
ways and storage area clean. All flammable material shall be stored in the designated storage
area, any spill shall be cleaned as soon as possible to prevent a slip / fall or fire hazard.

8. CHEMICAL SAFETY PLAN


All Chemicals shall be stored in a well-ventilated protected storage area. All plant workers shall be
trained appropriately for the safe handling of chemicals, and using all required safety equipment
like Safety Goggles, Rubber Gloves, Safety Shoes and mask & Splash Suit if required. MSDS of all
chemicals stored at plant shall be available at site.
In case of any spillage, the spill material shall be cleaned appropriately, as soon as possible, using
spillage control material provided.

9. TRAINING
Plant employees shall be trained for:

 Appropriate use of PPEs as per nature of work


 How to use the fire extinguishers
 How to prevent and / or control chemical spills
 How to assist employees on evacuating the Plant in the event of an emergency.
 How to provide first aid in case of any electrical, chemical or accidental injury
 Necessary safety instructions / precautions

10. MEDICAL EMERGENCY:


In case of any medical emergency, report the incident to the project / plant supervisor and HSE
In charge as soon as possible. Use telephone, if possible, to inform Plant / Project site Emergency
Management at the given Emergency Telephone Numbers. Give the location of the emergency,
employees name, type and extent of the injury. Request assistance if needed.

 Take the necessary steps to comfort the victim.


 If immediate threat to life of injured employee is imminent, render first aid as per
requirement.
 If the victim can be moved they should be loaded into a vehicle and transported to the
nearest medical facility.
 Victims who are not in a life threatening situation may elect to be transported to a facility
of their choice.
 Inform FTI Management about the incidence at the given FTI Emergency Numbers.

11. EMERGENCY ACTIONS AS PER DIFFERENT EMEGENCY CONDITIONS:

Project In charge / HSE In charge of site is / are responsible to assure that emergency situations
shall be handled as per the procedure given below accordingly.

11.1. IMMEDIATE RESPONSE IN CASE OF MINOR INJURIES:


In case of minor injury the immediate response is the obligation of FTI. If any minor injury
occur than the present shift supervisor is responsible to take immediate action and provide
essential first aid to the victim/ injured person.
11.2. FIRST AID PROCEDURE IN CASE OF ELECTRIC SHOCK:

1. Look first. Don’t touch. The person may still be in contact with the electrical source.
Touching the person may pass the current through you.
2. Call or have someone else call on Emergency Telephone Numbers of Project / Plant site.
3. Turn off the source of electricity if possible. If not, move the source away from you and
the affected person using a non-conducting object made of cardboard, plastic or wood.
4. Once the person is free of the source of electricity, check the person's breathing and
pulse. If either has stopped or seems dangerously slow or shallow, begin Cardiopulmonary
Resuscitation (CPR) immediately.
5. If the person is faint or pale or shows other signs of shock, lay the person down with the
head slightly lower than the trunk of his or her body and the legs elevated.
6. Don’t touch burns, break blisters, or remove burned clothing. Electrical shock may cause
burns inside the body, so be sure the person shall be taken to a medical facility / doctor.

11.3. FIRST AID IN CASE OF CHEMICAL SPLASH IN THE EYE


If a chemical splashes into your eye, take these steps immediately.
7. Flush your eye with water. Use clean, lukewarm tap water for at least 20 minutes. Use
whichever of these approaches is quickest:
8. Put your head down and turn it to the side. Then hold the lids of your affected eye open
under a gently running faucet. If you have access to a work site eye-rinse station, use it.
9. If chemical splash is spread to some other parts of body, get into the shower and aim a
gentle stream of water on your forehead over your affected eye. Or direct the stream on
the bridge of your nose if both eyes are affected. Hold the lids of your affected eye or
eyes open.
10. Wash your hands with soap and water. Thoroughly rinse your hands to be sure no
chemical or soap is left on them.
CAUTION

 Don't rub the eye — this may cause further damage.


 Don't put anything except water or normal saline rinse in the eye. And don't use eye drops
unless emergency personnel tell you to do so.

11.4. USING A FIRE EXTINGUISHER


The following steps should be followed when responding to incipient stage fire:

 Sound the fire alarm and call the fire department at the given number, if appropriate.
 Identify a safe evacuation path before approaching the fire. Do not allow the fire, heat, or
smoke to come between you and your evacuation path.
 Select the appropriate type of fire extinguisher.
 Discharge the extinguisher within its effective range using the P.A.S.S. technique (Pull, Aim,
Squeeze, and Sweep).
 Back away from an extinguished fire in case it flames up again.
 Evacuate immediately if the extinguisher is empty and the fire is not out.
 Evacuate immediately if the fire progresses beyond the incipient stage.
 Most fire extinguishers operate using the following P.A.S.S. technique:
1. PULL... Pull the pin. This will also break the tamper seal.
2. AIM... Aim low, pointing the extinguisher
nozzle (or its horn or hose) at the base of the
fire.
3. SQUEEZE... Squeeze the handle to release
the extinguishing agent.
4. SWEEP... Sweep from side to side at the base
of the fire until it appears to be out. Watch the
area. If the fire re-ignites, repeat steps 2 - 4.
12. IMMEDIATE RESPONSE IN CASE OF SERIOUS INJURIES:
In case of serious injury immediate first aid will be provide to victim and in the meantime
call to the project / plant site emergency number for the their ambulance facility or call to
given ambulance contact number.
The ambulance will drop the victim to the nearest medical facility, where necessary medical
aid may be provided.
