This has reference to the incident which transpired on December 12, 2019 at around 0530H.

On the above date and time, a report was made as regards an incident involving an accident where shuttle service
SV41 with you as driver hit a part of the “labangan” as you were maneuvering the shuttle service on reverse. This
resulted in scratches and dents on the right side back bumper and a broken glass on the right side tail light of SV41.
As the driver, you were responsible for the incident.
Your actions and omissions are clear manifestations of GROSS NEGLECT OF DUTY WITH MINIMAL DAMAGE a LESS
SERIOUS OFFENSE as defined under our Company Policies and Procedures memorandum 135-3
You are hereby directed to explain in writing within Six (6) days from receipt of this notice why you should not be
dealt with for the above offense. Failure to submit your written explanation shall be deemed a waiver on your part
and we shall be constrained to take the necessary action based on evidence on record.
