To whom it may concern,

I am writing to you to request that my son, Cai, Zihong of 6-Sobriety to be

excused temporarily from school from October 15 or October 18 (his flight)
to indefinite date. He needs to undergo surgery for Nasal Polyposis and I
have been advised by his Doctor that he need to be treated on China for
the best surgery.

I apologize for my son’s absence but I hope you understand that his illness
has rendered him unable to attend school at this stage.

Should he miss any work, assignments, etc, please let me know and I will
happily make arrangements for this to be made up in due course.

Attached is the photocopy of his medical result and his doctor

recommendation.
