Dear Sir / Madam.

Through this letter, I would like to inform you that as of today's, I can not be present in the office
to work as task and responsibilities, because I had to rest at home as a doctor's advice for a
disease that I suffered, following doctor's letter examination results which I attach.

After recovering my health, I'll go back to work as task and my responsibility as usual. Thank
you for all the attention.
