Job Summary:
This position is responsible for assisting in the delivery of community mobilization, PMTCT services,
RH, HCT Services, improving retention, intra/inter-facility referrals in supported health facilities
overseen by the organization in order to achieve organizational goals. Also, he/she would oversight
Case Management and Key Population activities.
The GF’ Optimizing Investment for Impact in HIV and TB in Nigeria will help reduce the HIV and TB
burden in Nigeria where PHRI will be implementing the grant in 5 states.
Prime Health Response Initiative Formerly known as Hygeia Foundation is implementing Global Fund
to fight AIDS, TB and Malaria, (GFATM) Project under the New Funding Model implementation in 5
states as Sub-recipient (SR) under the Family Health International (FHI360) who is the Principal
Recipient (PR) for HIV/AIDS component of the Global Fund.
PHRI is charged with the responsibility to provide Treatment Adherence, Care and Support services
to clients in all Global Fund supported facilities in 5 States which are; Osun, Ondo, Oyo, Ekiti and Ogun
states.
Job Description:
• Elicits and document contacts of all index clients assigned to the case manager.
• Ensures HTS are provided for all eligible contacts of index clients.
• Ensures enrolment and initiation of all positive contacts of index clients
• Provides treatment education, adherence counselling and peer support to assigned clients at
health facilities and community following the adherence support schedule.
• Facilitates active referrals of clients to treatment through escort services.
• Maintains a log of all assigned clients contacts (descriptive addresses and phone numbers) and
keeps comprehensive records of clinic appointments and interventions carried out to support
them on retention.
• Maintain confidentiality, respect privacy, and preserve the clients’ routine and independence as
much as possible.
Qualification
• A minimum of OND in any field
• Case managers should be literate in both English language and local dialect.
• They should preferably be expert clients and /or social workers and retired health workers who
have relevant knowledge and skills and can function as case managers.
• They will also be resident in the communities/LGA where the health facility is domiciled.
• Good computer knowledge is an added advantage especially Microsoft applications
