INFORMATION

The undersigned Prosecutor hereby accuses JHAZEEL ZHAN JEBONE

of the crime of VIOLATION OF B.P. BLG. 22 committed as follows:

That on or about the 25th day of December 2018, in the City of Dipolog,
within the jurisdiction of this Honorable Court, the said accused did then and there
willfully, unlawfully and feloniously and knowingly, make, draw and issue Check
No. 00023456 in the amount of TWO HUNDRED FIFTY THOUSAND PESOS
(P250,000), Philippine Currency, drawn against Banco De Oro Bank Dipolog
Branch, and payable to “DOREENA ARANAL” in payment of her balance of the
purchased motor vehicle; that the accused knew fully well at the time of the
issuance of said check that she did not have sufficient funds in or credit with the
drawee bank for the payment of said check in full upon presentment; that upon
presentation of said check to the bank for payment, the same was dishonored and
refused payment for reason that the check was drawn against an “INSUFFICIENT
FUND”; and that despite receipt of notice of such dishonor, the accused failed to
pay said complainant the face amount of said check or to make arrangement for the
payment in full of the same within five (5) banking days from receipt of the said
notice, to the damage and prejudice of DOREENA ARANAL.

CONTRARY TO LAW (Violation of B.P. Blg. 22).

INFORMATION

The undersigned Prosecutor hereby accuses JAYMARK BALBOSA of the

crime of HOMICIDE committed as follows:

That in the evening of March 9, 2019 in the Municipality of Siocon,

Zamboanga Del Norte, within the jurisdiction of this Honorable Court, the said
accused, armed with a gun, with intent to kill, without justifiable cause and without
sufficient provocation, did then and there willfully, unlawfully and feloniously
attack, assault and shoot LOWEL DAVE MANUEL, alias “gahi” thereby
inflicting upon the latter gunshot wounds in the different parts of his body which
caused his instantaneous death; that as a result of the commission of the said crime,
the heirs of the herein victim suffered damages in the amount to be proven during
trial.

CONTRARY TO LAW (Violation of Art.249 of the Revised Penal Code).
