AFFIDAVIT OF SELF ADJUDICATION

I, FELICIDAD DELA CRUZ VILA, Filipino, of legal age married to

Jesus A. Vila, with residence ad postal address at #143 Dr. Sixto Antonio
Avenue, Maybunga, Pasig City, after having been sworn to in accordance with
law, hereby depose and say, that:
1. I am the only surviving heir of my mother, Agapita Santos dela Cruz
who died intestate in the municipality of San Juan, Metro Manila on July 25,
1999 as evidenced by her Death Certificate issued by the Office of the Civil
Registrar General, photocopy of which is hereto attached as Annex "A" and
made an integral part of this Affidavit;
2. The said deceased left the following property, more particularly
described as follows:

a) TRANSFER CERTIFICATE OF
TITLE NO. 32307
“A parcel of land (Lot 2-C of the subd. plan
(LRC) Psd 108933, being a portion of Lot 2, Psu-
87035, LRC Rec. No. 42453), situated in the Bo.
of Maybunga, Mp. of Pasig, Province of Rizal,
Island of Luzon. xxx xxx xxx containing an area of
TWO HUNDRED ONE (201) SQUARE METERS,
more or less. xxx xxx xxx”
b) "Lot ABCD, Block 664, Section Court of
Salvation-Phase II Premium, being a portion of
Transfer Certificate of Title No. 277820, situated
within the cemetery of Barrio San Dionisio,
Municipality of Paranaque, Province of Rizal, under
Contract No. 27863, Deed of Sale and Certificate of
Perpetual Care;
3. In pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby
adjudicate unto myself the above described real estate by means of this
Affidavit and hereby file the same with the Register of Deeds for the City of
Pasig, Metro Manila with the request that said adjudication be made effective
without judicial proceedings as prescribed by the aforementioned Rules of
Court.

AFFIDAVIT OF SELF ADJUDICATION

I, REGINA C. JUINIO, Filipino, of legal age, single with residence

ad postal address at #11 E-Jacinto St., Malinao, Pasig City, after having been
sworn to in accordance with law, hereby depose and say, that:
1. I am the child and only surviving compulsory heir of LEONA C.
JUINIO who died intestate in Pasig City on August 27, 2012 as evidenced by
her Death Certificate issued by the Office of the Civil Registrar General,
photocopy of which is hereto attached as Annex "A" and VICENTE A JUINIO
who died intestate in Pasig City on August 1, 2002 as evidenced by his Death
Certificate issued by the Office of the Civil Registrar General, photocopy of
which is hereto attached as Annex "B" made an integral part of this Affidavit;
2. My deceased parents left an account at Banco De Oro as well the
Original Owner’s Copy of Property Lot, covered by Transfer Certificate of Title
as follows:

A) TRANSFER CERTIFICATE OF
TITLE NO. PT-91937
B) TRANSFER CERTIFICATE OF
TITLE NO. PT-91116 and;
c) TRANSFER CERTIFICATE OF
TITLE NO. PT-91936

3. In pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby

adjudicate unto myself the above bank account and described real estate by
means of this Affidavit and hereby file the same with the Register of Deeds for
the City of Pasig, Metro Manila with the request that said adjudication be
made effective without judicial proceedings as prescribed by the
aforementioned Rules of Court.
