and installation of internet service by Philippine Long Distance Telephone ("PLOT") under my
property premises at San Alejandro , Santa Maria, Pangasinan . He is given this authorization so
that you can readily assist his application anytime soon.

This authorization letter is requested by the Tenant for whatever purpose it may serve .
