I, Dianne P. Rabanes after having been duly sworn to in accordance with

law, do hereby depose and say:

1. That I am the duly elected Treasurer of the Októ Agricultural Co-operative to

act as such until my successor shall have been appointed and qualified in
accordance with the By-laws of the Cooperative;
2. That as such, I hereby certify that the authorized share capital of the
cooperative is Four hundred Eighty thousand PESOS (Php 480,000.00);
3. That the subscribed share capital of the cooperative is One hundred twenty
thousand PESOS (Php 120,000.00) which is at least twenty five (25%) percent
of the authorized capital;
4. That the paid-up share capital of the cooperative is Thirty thousand PESOS
(Php 30,000.00) which is at least twenty five (25%) percent of the subscribed
capital;
5. That the total membership fees paid is Fifteen thousand PESOS (Php 15, 000).
Provided, each member paid One thousand PESOS (Php 1, 000); and
6. That I have actually receive the total paid-up share capital and membership
fee.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 15th day
of September, 2019 in Max Suniel, Carmen, Cagayan de Oro City, Misamis
