AFFIDAVIT OF LOSS

I, ROSEMIN H. ABAY, of legal age, Filipino, married with

residence and postal address at Lilly of the Valley, Calumpang, General
Santos City, after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am a holder of a Locker ID issued by Philbest Canning

Corporation, Tambler, General Santos City;

2. That sometime in March 2012 said Locker ID was lost while

inside my bag and diligent efforts exerted to locate the same
proved futile and is now considered lost;

3. That I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever purpose this may legally
serve.
