This graph shows that the month August until October have the highest recorded incidents in
Lost time Injuries. There are all recorded incidents in Fist Aid Incidents, Medical Treatment Injury, Lost
Time Injuries, Near Miss Incident and Lost Time Injury Frequency Rate. There weren’t any records in
Incidence Rate. Most of the whole year there have been Lost Time Injury recorded.

This table shows that the most involved with incidents are the customer service staffs and the
major incidents recorded were in Lost Time Injury with 46% based on the WHS data. The next one is the
forklift drivers under the MTI and LTI. There once incident happened with the driver and a customer.

2. Develop an evaluation report. Include in your report:


a. three recommendations, including improvements to recordkeeping practices and
induction or training
 Trainings on Customer Service Staff
 Machinery and equipment preventive maintenance on Forklifts
 Pre- employment screening

b. justification for recommendations with reference to internal and external


requirements, such as WHS legislation, standards, codes of practice. Refer to
relevant sections of legislation.
 Training and Coaching
The WHS data show that the most commonly to get incidents are the customer
service staffs, possibly that the staff haven’t got any training.
The training that should be implied is ‘’Manual Handling- Safe Lifting Technique
Training’’, the incidents are more likely due to failure of demonstrating the
equipment or lifting the equipment or tools. The training must be done every
month and must be monitored each staff how they are done well with the
trainings as well as coaching. We also need to implement ‘’ WHS Training and
OHS Training’’ to let all the staff safety awareness and maintaining a safe work
environment. This also can cover a healthy lifestyle implementing for the
employees which recommend each of them to exercise everyday and eat
healthy food.
 Machinery and Equipment preventive Maintenance
It was recorded that the forklift drivers complained about the fumes coming
from the forklifts and there weren’t any actions implied until on that next
month a driver found unconscious. The only way we can prevent this kind of
situation is to hire a maintenance who checks the vehicles every fortnight or if
needed.
 Pre-employment screening
With the WHS data, there have been an employee who gone psychotic
behaviour and physically threatens another employee. To prevent this kind of
situation, there must be pre- employment screening to the employees to make
sure everyone is in their good condition in the workplace.
3. Create a plan for the implementation of your recommendations, including the
implementation of training/induction recommendations. Ensure you:
a. include regular review and reporting
b. include timelines, resources and actions
c. are consistent with recommendations and justification with respect to compliance
and adherence to organisational requirements.
