AFFIDAVIT

I, FERNANDO CALVIN B. PEREZ, of legal age, Filipino,

single and with residence and postal address at Blk 15 Lot 20
Phase 2B Mabuhay Homes 2000, Pantok, Binangonan, Rizal
after being sworn in accordance with law depose and state
THAT:

1. I am the owner of the compact disk (cd) containing the

closed circuit television (CCTV) footage dated March 25, 2019
taken by my CCTV located in front of my house at my above-
given address;

2. I voluntarily gave said CD to Fidela D. De Leon of Blk

15 Lot 12 Phase 2B Mabuhay Homes 2000, Pantok,
Binangonan, Rizal to shed light on what transpired on March
25, 2019 which was captured by my CCTV;
 3. I have executed this affidavit on my own volition,
without threat or intimidation from anybody, to attest to the
foregoing and for whatever legal purpose it may serve.
