AFFIDAVIT OF SUPPORT
for TRAVEL EXPENSES

I, AMALIA SALVA QUILANG, of legal age, Filipino citizen, widow and residing at
Ugac Sur, Tuguegarao City, Cagayan, after being first sworn according to law, hereby depose
and say, that:

1. I am the biological mother of DIONNE NATHANIEL PERALTA SALVA who was born
on the 31st of August 1993 and SHEENA MAE SALVA QUILANG who was born on
1st day of October 1994 which my children is also a holder of passport bearing no/s.
P5481818A and P5488622A, respectively;

2. We will travel to Japan on August 26 up to August 30 2019, which I will be

responsible for my children’s accommodation, subsistence, and all other expenses
for the said travel;

3. I guarantee the above-mentioned will not violate any laws of the said country, or
work or seek employment;

4. We the above-mentioned will return to the Philippines on or before the expiration of

the period of their authorized stay in Japan. I understand that overstaying, working
without a valid permit and pimping are crimes in the said countries;

5. I execute this Affidavit to attest the veracity of the foregoing and in support for the
travel expenses of the my children and for whatever lawful purpose it may serve.

IN WITNESS WHEREOF, I hereby affixed my signature this 9 th day of July 2019 in

Tuguegarao City, Cagayan.
