I, JOVY C. MELANO, of legal age, Filipino, and a

resident of Calle Posporo, Barangay Vitali, Zamboanga
City, after having been duly sworn to in accordance
with law hereby depose and say:

1. That I am a holder of the Automated Card

(ATM) issued by Landbank of the Philippines

2. That the Automated Card (ATM) was totally gutted

by the fire during the incident that transpired
last March 17, 2018;

3. That I am executing this affidavit to attest to

the veracity of the foregoing facts and for
whatever legal purposes it may serve within the
bounds of the law.
