CERTIFICATION
In connection with my Teacher-Application with the Department of
Education (DepEd) through the division of city schools of Northern
Samar.
I Mr. Gerardo Alcazar Megenio Jr, the undersigned, a Filipino
citizen, of legal age, with permanent address at Brgy.Dalakit
Catarman, Northern Samar, after having been sworn in accordance with
the existing laws, hereby depose and state:
That, I have submitted the following documents to the afore mentioned
office, to wit:
A. One (1) copy of CSC FORM 212(2017 VERSION) with latest Passport
size I.D picture;
B.PRC-Certified True Copy of the following documents:
B.1. Identification Card;
B.2. Certificate of Ratings;
B.3. Certificate of Passing
C. One (1) copy of School-certified True copy of my Transcript of
school records.
D. One (1) Copy of School-certified True copy of my General weighted
average.
E. One (1) copy of each of my Certificate(s) of Specialized Training.
F. One (1) copy of Voters certificate and/or any proof of residency,
as deemed acceptable by the School Screening Committee; and
G. One (1) copy of latest NBI Clearance.
