requirements for this degree, I am required to undergo the Practicum/On-The-Job Training
(OJT)/Internship for a minimum of 150 hours starting June 2019 – July 2019.

STUDENT AGREEMENT AND LIABILITY WAIVER

KNOW ALL MEN BY THESE PRESENTS:

This Student Agreement and Liability Waiver made and executed this June 2019 at
University Site, Lucena City by Joy C. Valdoria, Filipino, and resident of 93 Quezon Avenue
St., Brgy. 2, Sariaya, Quezon and her mother Alicia C. Valdoria, of legal age and resident of 93
Quezon Avenue St., Brgy. 2, Sariaya, Quezon.

Witnesseth

THAT above-named student will undergo an internship program with the CENTER FOR
WOMEN AND CHILD AT RISK IN NEED OF SPECIAL PROTECTION for a maximum
of 150 hours as part of her academic requirements.

THAT the University is granting permission for the student to undergo said practical training at
the above-mentioned institution subject to the following terms and conditions:

1. During the period of his/her community volunteerism, the University shall not provide
financial assistance to the student;

2. The Student shall abide by all rules and regulations imposed by the practicum professor
and the University and shall exercise care and diligence in the performance of his/her
tasks during the internship program;

3. The University shall not in any way be held liable for injuries and/or accidents the
student may suffer during the duration of his/her internship;

4. The University shall be held free from any liability for damage or injury to third parties
which may be due to the intentional or negligent acts of the Student;

5. This agreement and liability waiver shall be in effect from June 2019 – July 2019.

This is to inform you that permission is hereby granted to our daughter, Joy C. Valdoria to
participate in the Practicum/On-The-Job Training as a requirement of the Old College
Curriculum in the Bachelor of Science in Psychology program of the College of Arts and
Sciences.

We understand that the said training will cover 150 training hours, which will be conducted in
the Center for Women and Child at Risk in Need of Special Protection located at Lucban,
Quezon from June 2019 – July 2019.

We understand further that the Faculty Coordinator-in-charge will monitor the performance of
the student through site visit and that an immediate supervisor within the institution will take all
the necessary precautions to keep my daughter safe.

my full consent so that she can graduate from her program.

TRAINING AGREEMENT

Our agency/institution/establishment through the undersigned department Acting Mayor

Hon. Armando V. Abutal, MSWDO Ms. Mutya M. Rada, RSW and Center Head Ms.
Josephine J. Robelo, RSW hereby submits to the following training provisions with the
COLLEGE OF ARTS AND SCIENCES through its Dean as follows:

1. That the CAS student trainee, Joy C. Valdoria, will report for work on the following schedule:

2. That the above-mentioned CAS student-trainee shall perform the following tasks in our
agency/institution/establishment:

ETHICAL AND LEGAL STANDARDS, INDIVIDUAL AND CULTURAL DIVERSITY,

PROFESSIONAL VALUES, ATTITUDES, AND BEHAVIORS, COMMUNICATION,
INTERPERSONAL SKILLS, AND SUPERVISION, PROFILING, TEST
ADMINISTRATION, TEST EVALUATION, SENSITIVITY TRAINING AND GROUP
DYNAMICS

3. That the above-mentioned CAS student-trainee is not allowed to work on overtime or during
holidays, Saturdays and Sundays.

4. That our agency/institution/establishment thru the undersigned department head/student trainer

takes responsibility of the above-mentioned CAS student under the “principle of loco parentis”.

5. That our agency/institution/establishment through the undersigned department head/student

trainer takes responsibility of the above-mentioned CAS student while he/she is on scheduled
duty or performing official functions/work outside the confines of our office under the “principle
of loco parentis”.

6. That our agency/institution/establishment through its department head/student trainer shall

comply with school evaluation requirements for student trainee/s.
