AFFIDAVIT OF VOLUNTARY SURRENDER

That I, JOHN A. DOE, of legal age, Filipino, single and a resident of Sto.
Cristo, Sampaloc, after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am the accused in the alleged murder of Darla Starla in relation

to the Complaint-Affidavit filed by Ana Imperial, pending before the
Office of the Prosecutor, City of Manila;

2. That I hereby voluntarily submit myself, thus, for all legal intents and
purposes, I hereby manifest my voluntary surrender hereof;

3. That I have executed this affidavit for the information of the proper
authorities, and for whatever legal purpose it may serve.
